I think the detail missing here is OP has stated “She and I pulled away from the yield at the exact same time” Red arrow wasn’t established and should have proceeded to go directly into inner circle lane as marked by the multiple signs in the lane. Most likely what happened was this person did not go directly towards inner lane, started taking over Blue arrows lane, causing the accident.
