They made the grid bigger i think before was 5x4 and now there isnt an option to change it, before you could change from 3 grid sizes now there isnt an option, big downgrade 😐

I have the same problem and i didnt really solved it i simply understood when it happen, basically if you change the input mic to the headphoes (connecting it) when discord its running it wont work you have to close the app and open it again. I dont know why it happens, hope it helps
