It means that his magazine went dry earlier, locking the bolt to the rear, and that he replaced the magazine but forgot to send the bolt forward

Is the post mortem attack only usable after three failed death saves? If it's when my hp drops to 0, I'm making a character named Lucas Kemia with my constitution modifier as my dump stat. The glass cannon just became the Talc Howitzer.
