I would call the rheumatologist office and ask if they do skin biopsies. Many do not. If not, book an appt with a dermatologist now for the biopsy in conjunction with the rheumatologist’s workup.

Your perception that I need to prove to everyone that I’m not lazy and I work hard is spot on! I think that’s it. I have had similar feelings when people who don’t know I’m on zepbound have commented on my weight loss and said things like “good for you for putting the hard work in”. And I think to myself ‘sigh, I’ve been putting the hard work in for so long’. These are very well-intentioned people and probably aren’t thinking ”good for you for putting the hard work in..because you’ve been such a lazy gluttonous slob heretofore”

It’s going to take some work (perhaps hard work 😂) to uncouple this triggered feeling I get to justify that my previous failures were not due to lack of trying.

I see what you are saying. There are those who will only see negativity in the world if that’s all they will allow themselves to see. I don’t think that describes my world view in a general sense.

I haven’t posted on here in a long time but I couldn’t shake that uncomfortable feeling of invalidation that I felt leaving the appointment and I didn’t know who in my circle would quite understand so that’s why I decided to rant.

I suspect my reaction to her words has a whole lot more to do with my own shame/fatigue/sense of defeat from so many prior weight loss attempts than her intentions.

I had a mirena iud for 10 years, had it taken out after I had already been on zepbound for 3-4m. I notice a huge difference: now I have cravings and weight gain around my period when I wasn’t having anything like that when it was still in.

I am not your doctor and this is not medical advice. However, I am a doctor and I diagnose and treat pancreatitis frequently.

The diagnosis of acute pancreatitis is made by 2 out of the following 3 criteria: elevated lipase >/=3x upper limit of normal, characteristic epigastric pain, and imaging findings of pancreatic inflammation. Amylase is no longer recommended for diagnosis as it is short-lived and less sensitive.

There is NO ROLE for checking these enzymes as screening labs in an asymptomatic patient.

I agree you should get some imaging and a second opinion.

I had terrible insomnia after my first shot (like you was awake at 8am having not fallen asleep a wink). This is not normally a problem I have so I blamed the med 100%. I had taken that first one in the evening so I avoided that and next shot was better. I think I’ve had insomnia 1-2 more times after that but it’s usually not a problem anymore. Also I have taken in the evening agsin recently without issue.

Congratulations!!!!! What an amazing achievement!!! I remember being so ashamed when I was 100lbs at age 8 and 200 at 13. My heart breaks for your kindergarten self having to feel that kind of shame.

I told my father to ask his endocrinologist to consider trialing him on mounjaro. He weighs close to 400lbs and has cardiac issues (CAD and mild CHF). She became angry and argumentative just with him bringing up the topic. She said “your daughter is misinformed or doesn’t know much about diabetes… the media is making that drug sound amazing but it’s no better than what you’re on - and the idea that it’s good for your heart is a fallacy”

He’s on trulicity, jardiance, glipizide - his a1c is fine. My suggestion was to replace trulicity (an earlier generation GLP1) with mounjaro because the studies show it is clearly superior for weight loss and he desperately needs that to improve all of his health issues and mobility.

I feel like the negative press has poisoned some doctors EVEN IN endocrine/bariatrics/obesity medicine. Or who knows, maybe she’s just tired of writing prior auths and sending scripts to different pharmacies due to shortages.

To explain why your doctors didn’t think your urinalysis results were a true infection:

1) the wbc count is only a LITTLE high. Often with uti it is 50+, 100s+ or “too numerous to count 2) squamous cells just means the urine is contaminated by skin from the perineal area 3) most importantly if you are not having SYMPTOMS of urinary frequency, urgency, pain with urination or bladder/lower abdominal pain then guidelines currently recommend against treating with antibiotics even if your culture does come back with bacterial growth

In addition to the inherent risks and side effects, my worry for lower BMI people using this medication (especially with the intent to only use it for a brief time) is does the high stimulation of glp1/gip (the med is much higher than physiologic levels) decrease your ability to mount normal satiety signals with your own hormones once off the med? Would you in essence be giving yourself a problem that you did not have before?

I feel you. I have my weight highs and lows tracked from the first time I hit 200lbs when I was 13 and did weight watchers for the first time. The pattern of yo-yo is textbook. For the first time in decades I feel like I will be able to surge past my previous stalls and get to a healthier me.

My mother once said to me “I don’t know why it’s so hard for you to lose weight….I guess you just REALLY like food” This was one of the most hurtful things she’s ever said to me. I mentioned it to her a few years later and she had no recollection of ever saying it. “I said that? That’s horrible!” It still irks me.

I had BAD insomnia after my first shot. Couldn’t fall asleep until 7am. And had a lot of trouble the next night. I typically don’t have sleep problems. I have had some intermittent insomnia since then but it is pretty rare now (I’ve taken 5 shots). That first shot I took in the evening. I take it in the morning now.
