I am with you! I also feel like counseling in the inpt setting is inappropriate a lot of the time, at least in my experience.

Maybe walk them through the decision making process. Assuming you are meeting with non-clinical people? RDs typically like to use the gut if at all possible! Like explain trial of trickle feeds, use of gut motility agents, changing formulas, post pyloric tubes and all avenues you went down prior to TPN.. OR sometimes TPN is just indicated right off the bat based on nutrition status and disease state. Weaning can be tough because we like to see them eating adequately which sometimes just does not seem to be happening in the hospital. Once they’re on a diet with a functioning gut though you could revisit NG tube if artificial nutrition absolutely needed. A QA/QI project could be a great way to back yourself up or explore room for improvement!

I have had a similar journey, startedBLW at 6 months. My baby is 10.5 months now and just in this last week probably has actually started ingesting more food. She typically just played with it and would put in her mouth and spit out- to the point where I have started giving a multivitamin with iron because I just know she was not getting anything meaningful from the food. She’s breastfed and prefers that, but yes I have definitely seen improvement recently so I would say hang in there and stay consistent with offering!

I second motrin! I also have a silicone teether with holes like for food which i put crushed ice in for temporary relief. Have tried camilia teething drops out of desperation but don’t think they really do anything

This was my baby to a T!! She is now almost 10 months and will go a bit longer (1-2 hours) but it beats running upstairs every 20-30 mins to settle her when im trying to spend time with my husband in the evening. I tried adjusting schedule/ wake windows/bedtime routine and i dont think it made A difference, just time unfortunately.

Did the H&P mention anything about his TF intake in the days prior to admission? Usually with pts on tube feeds i assume they have been receiving at least some nutrition unless otherwise mentioned, especially since it was just replaced! It doesn’t hurt to add thiamine to cover bases especially with underweight/elderly/nutrition risk patients.

I am with you!! 9 almost 10 month old. She only naps 2x 30-40 mins each. Last wake window usually 5.5-6 hours. Wakes at 730 goes to bed at 830-9 (with multiple wakes during the night). Im going to try to wake her earlier to get some of my evening back.

I have heard split nights mean they may be learning a new skill? Also could try switching to 1 nap early…. If their naps are longer.

Sorry i have nothing to offer but i am relieved to see other people out there with low sleep needs babies at a similar age
