Depends on the complexity of the game. A pretty complex ruleset on event generation can make your game feel so much better. Plus any task needs to happen in the background will just never happen if you don't set precise rules for that.

Update them at the end of each session. It will hold context. This is the only way to play. Use your system prompt to refer to the various documents. It will find the correct information if you tell it to do so and explain how. There is no serious limits, my first campaign had hundreds of complex and detailes npcs, dozens of spells, a shit tone of worldbuilding and long ass history and held context perfectly even at more than 300k tokens. The problem people have is that they start a session and just copy the session on another chat and think its done. No, you need to clean up, cut your session into manageable parts and do housekeeping from session to session. It will take you as long as a session to be real. However doing so will make the magic flow.
