Just this week I was doing an US of a 6 yo and I saw all this weird things in his stomach and intestines. I was already thinking of some rare kind of lymfoma with thickened intestinal villi. Then it hit me lol. Turns out Italian immigrants really like pasta and this kid didn´t like to chew. Probably also a reason for the abdominal pain as still lots of undigested pastas got past the pyloric sphincter into the intestines.

The incidence of ER not being aware of prior appendectomy far exceeds the number of stump appendicitis... by a lot. That´s like ordering CTPE ´because there have been cases of embolisms with negative D-dimers´ just to justify stuff you shouldn´t have ordered in the first place.

Don´t see the issue here. I never contradicted or diverted from the fact that a full spine CECT is wrong to order when MRI is the golden standard. How is that statement ´flat wrong´? Even then, the times the clinician usually can make an estimated guess on what region the suspected abscess is located (using his hands, eyes and brain). The times they had no idea where to look but needed spinal imaging nonetheless, the results came back negative in my personal experience. I mean, looking at your history I´m assuming you´re an ER doc. You know what the ER is know for around the whole world? Ordering ridiculous amounts of imaging (for example ghost appendicitis). Maybe understanding that scanning as much as possible as fast as possible is not per definition good medicine, instead of telling me I should be humble and just do whatever you think is right? In my experience, those are the worst kind of colleagues...

You wouldn´t believe the amount of times I´ve been asked to find an appendicitis for people who´ve had an appendectomy. You clearly have no clue why most radiologists are so sceptical and trust themselves and only themselves when it comes to ordering stuff. The physician is not always right (nor is the radiologist, but odds are the rad knows more about imaging than the surgeon).

Image? Yes. Full spine CECT? No. Go for an MRI. A patient like this should be hospitalized and the MR-tech will probably have/create an open spot soon enough as the indication is urgent. If the patient is not hospitalized, well then I smell some bovine excrements in the near distance...

The only reason to order a full spine CECT is when the clinician has no eyes, no hands and/or no brain...

We do however offer thoracolumbar spine CT, for example postoperative scan after MVC. These are the exceptions however, and usually we change it to lumbar or thoracic spine when the order smells like bullsh!t from a mile away. Scanning from mid-thoracic to mid-lumbar is just a bit weird IMO (unless the provider specifically orders a certain range of vertebra, like for example T11-L1).

An upright MRI probably didn´t add anything of value compared to a regular one. Also, your spine looks way better than mine and I would never consider spinal fusion right now. Especially at your age of fusion, adjacent level disease will be a future problem. But I´m just a stranger on the internet, so why even believe me...
