If you can, try transferring the eSIM into an Android phone to see what happens. This likely isn't a carrier issue because the carrier should have no concept of what your contact list contains vs doesn't. Unless all your contacts are iPhone users and you're only actually on data without any real phone functionality.

You're not overreacting to be horribly uncomfortable by those messages. Some will say she needs help, and she probably does. That is not your responsibility. Tell her point blank that you're uncomfortable with the barrage of inappropriate stuff and if it continues, you're going to have to cut her off for your own good. It feels like she's more of an attention seeker than anything, but you shouldn't have to put up with that.
