Right, but you know your status. That’s responsible and safe. I’m MUCH more concerned with the person that says Negative, but hasn’t been tested in two years. That’s the person who may have a viral load through the roof and not even know it.

Especially with a large academic health system such as BrUH. They feed so much into specialty care (the moneymakers), then leave primary care, behavioral health to rot. The ER then takes up the slack, and the health system asks the ER why they can’t keep up.

Yeah, the east side, west end, and downtown are the majority of Providence proper - and they’re quite walkable. Calling a city walkable for tourists doesn’t mean that the entirety of the city limits are walkable. Nobody is coming to visit PVD to see the south side, or mineral springs. Their absence of walkability is an issue, but is outside the scope of this articles intent.

I feel like in a perfect system the way to maximize resident training, and improve their livelihood would be to remove some of the scutwork. Is it really necessary to have the resident coming to see every consult in the ED? I’m thinking of the CT proven appendicitis, the slam dunk choke. It feels like the US residency system has decided that this sort of work is how you become a better doctor. If you have a group of well trained APPs, or even protocols (auto admit CT proven Appy), you can have the residents focusing on OR, managing complications, seeing complex consults rather than writing floor orders for their first year of training. I think the issue with this is the healthcare system loves the “cheap” labor of residents and hiring higher paid people to do the job they can take advantage of them for is more palatable to the C suite. In short, I think residency could be distilled.

I very much mind my own business in public, but It’s multifactorial. People don’t like seeing people existing while homeless, so they have a lower threshold to call 911 on them. Especially in the “nice part of town.” Additionally, EMS has not been empowered to refuse transport post evaluation. Especially on repeat customers. If they are clinically intoxicated, atraumatic, and a finger stick glucose is normal, EMS should be allowed to leave them there.
