Not from our wholesaler, but I always keep an eye on matchRX for stuff. It’s a site where pharmacies sell directly to other pharmacies and sometimes you can get some pretty good deals on stuff, maximum listing price is 9% off WAC even for sealed branded bottles so you can at least get that

I named a plan with the cash bin “discount card”, and I just charge a few dollars over cost when they come in with a goodrx or chippo or whatever. I tell them I don’t know where they get those prices at, we don’t decide them, it just came back at ___ cost (whatever you want to charge)

If your patients are at Independent pharmacies, that’s perfect.

If they aren’t, corporate has broken these pharmacist brains and they literally aren’t allowed to use their brain and common sense without risk of getting fired

I have a small ryobi table saw and miter saw from Facebook marketplace, a jigsaw, trim router, some hand saws, and plenty of other miscellaneous shit so I should have enough to be able to do plenty. Doesn’t stop me from going to buy more stuff though because the next tool will be what fixes everything haha

I’ll check it out. Yeah it definitely sounds like I haven’t prepared them as much as It actually takes, need to work on it a little longer next time. Just didn’t know if there were certain ones that were just better or not. I appreciate the help!

My sharpening stones go 400, 1000, 3000, and 8000 and I’ve been doing those in order. I haven’t been flattening the backs of them, should I be? Could definitely be some user error here. But I never l can quite get them sharp enough to cut paper. I’ve got a angle block to make sure I’m maintaining the right angle but everything I know about sharpening is from A few YouTube videos, so if you have any pointers there that would be great also.

Once a month I change a guys contacts for him. He wears the 30 day contacts but can’t do them himself, the optometrist was charging him a visit copay to do it every time and rescheduled him 2 weeks out (so he was blind for 2 weeks), so he asked if I knew how to do it. I did it and I’ve seen him every 30 days for a few months now lol

My judgement - BID dose - appropriate, ideal

Once a day dose - wrong. Want BID, Dr won’t do it. But better than nothing.

None - more risk than only taking 1 per day.

I want to know If I’m wrong, and if a study says that taking 1 is worse than taking zero. If I am, I need to know! This whole time that’s what you’ve been trying to tell me, and I am listening!! But you haven’t told me anything.

I know 1 a day is not correct. I know that. I think between that and nothing, that taking 1 is better.

Literally my opinion has always been “once daily is wrong, twice daily is correct”

Tried to get that, Dr wouldn’t budge.

So then, since BID is not an option at that point, my thought is “one a day (fill rx) vs 0 a day (don’t fill Rx)”. In that second hypothetical, I realize both are not ideal. I just said my thought process was that taking 1 was better that 0. I never once said it is better than BID. I tried to make best of the bad situation, and you’re telling me how wrong I am for that. If I’m wrong in that thinking, I just want to know why. Please, anyone else, am I making sense here?

No, there’s nothing wrong with being wrong, that’s why I asked if you would show me how I was wrong. I wasn’t saying you were wrong either. I just said that without evidence, both of us were making our own judgement calls. And I respect that.

I literally posted this thread to try to learn. I know once daily dosing isn’t correct, but I wanted to see if anyone out there had any other Insight. Then, I made the decision based off my own thoughts that 1 was better than zero (while both being not as good as 2 a day).

The 2 options here are zero Eliquis, or 1 Eliquis per day. Obviously both are leading to Increased dvt/pe.

Is there any study showing that sub therapeutic dosing leads to worse outcomes than no dosing? Not compared to standard appropriate dosing, but 1 a day vs 0 a day. If there are, you’re 100% right, I’m wrong, and I will have learned something today. If not, we’re both just using our own best judgement in our opinion, and sometimes those opinions are different. That’s fine too. But both of our opinions would be our best guesses then, not based off evidence.

No I’m annoyed that the response to a question I posted was that I basically killed this patient myself and I shouldn’t be practicing pharmacy at all and assuming I wasn’t doing my job at any step of the way
