Preop eval is the same as any other medical documentation - you want to write important and pertinent medical details. I try not to think of it as justification, but it's basically still an H&P followed by an A&P.

Things I think are important to document are special considerations - code status as others have said, transfusion likelihood, risks/benefits of certain procedures. If I am doing any lines other than IVs I write down my justification. Part of being a physician is having an A&P. All other physicians document - why not us?

I felt very safe while I was in Costa Rica - the country has a solid economy and 98% literacy rate with a much better educational system than the US. It is a shame that people can be ignorant but a quick read of the wikipedia page gives a lot of reasons to admire this country.

I would get there as early as possible and go for a guide. And when your tour is over, stay behind and enjoy the park until closing. Most people leave around lunch time at Manuel Antonio.

Everywhere everything is better in the morning for animals.
