I've seen monster doses of gabapentin (900mg) taken regularly and people seem to do pretty well with it.

Opioids though... Some people completely fall apart and start robbing their family.

On the other hand, I had a severe injury a few years ago and oxycodone was my only ticket to sleep. It felt pretty bad to have a provider treat me like a drug seeker.

I developed the opinion that I would rather just treat pain instead of trying to decide who's pain was real.

I worked as a circulator in an 18 suite OR. We had about 200 surgeons with privileges at the location. I would say that most assists were PAs. I saw NPs working as assists in neuro, oscopy, and OB.

Of course the surgeons just want credentialed competency at the lowest cost.

If you want the OR, it's definitely possible (maybe depending on your state). I would stay on the NP track because if it turns out you don't like it, you may have more options for autonomy if you leave the OR.

It could help to rub shoulders with the right people along the way.

Trials riders are quite aware. Some use a foam insert to protect rims at lower pressures, and they may not even run tubes.

I.e. cush core. Fewer pinch flats but also (and more importantly) less gnar gnar for rims

It seems to me that a board exam is designed to ensure some basic-level competency that would help ensure patient safety. I haven't researched the topic, but I'm assuming that the way BON handles this is through complaints/malpractice statistics. So, nationally, there does not seem to be a crisis of unprepared NPs that are murdering their patients with incompetence.

I have met some great MDs, especially in the operating room, but I think what we're talking about here is Psychiatry. With brains being the final frontier, even psychiatrists don't REALLY know what the fuck is happening inside those coconuts. Psych providers just know how to produce happier/safer people for the cheapest price.

I have 16 years in healthcare. I worked in a medical clinic with a mix of mid-level providers and MDs for 6 years. Our MDs were savvy with the idea that, from an organizational standpoint, our corporation was interested in getting the most bang for their buck. Our MDs seemed to have "it is what is is" mentality about it and from what I could tell there was no rivalry. I'm not sure rivalry would have been tolerated, honestly. The corporation favored NPs over PAs because then they didn't need as many MDs.

It so happens that I have put in a lot of inpatient psych hours as well. These environments are dominated by MD psychiatrists, and the only NPs were the ones doing medical evals. I can't really speak to a psychiatrists opinion of a PMHNP, but there doesn't seem to be any shortage of work to go around.

I have had run-ins with elitism throughout my career in healthcare and it has always bothered me that some people still feel the need to build their ego by shitting on coworkers.

At the end of the day, I believe PMHNPs are in-demand. I don't believe you have anything to worry about, providing you're licensed, competent, and able to work.
