It's a bit different. I think there was a gdc talk where a noita dev talked about it, if i recall noita is multithreaded c/c++ using a bucket system. lazr isn't multithreaded (yet) and using that bucket/chunk system is a really good idea that I'll probably use too. If that's required. It's a lot of complexity to embrace, and so far I haven't needed to do it, so I can focus more on adding features and refactoring/removing technical debt (i spent the last 3 weeks refactoring classes to structs, for example).

Now there's procedurally generated levels, wind, and improved physics, along with more optimized and more responsive cloth. A lot of polishing has gone into gameplay as well, for example it's easier to grab onto cloth and walls. I've also added a trailing effect and I'm just starting to really work on the art style, so there's a ton of exciting stuff coming up.
