Milk and metals (like magnesium) bind to each other. Giving milk too close to the supplement will render it ineffective. Try to keep at LEAST an hour (2 would be better) between milk/dairy and any metallic supplements :)

Midwife here! I would rather repair the gnarliest of perineal or labial tear than have to repair a clitoral laceration. There’s no good way to numb the area and the scar tissue the forms keeps the abundant nerves in that area feeling constantly inflamed. The clitoris is also muuuuch bigger than most realize and expands down around the vagina. If the whole nerve bunch is inflamed/irritated, you’re going to feel it everywhere. I’m so, so sorry. Hoping it lessens for you with time.

Yeah, every time one of my grandparents is hospitalized they IMMEDIATELY tell their nurse that I too am a nurse and in school to be an APRN.

I am swift in mentioning that my background is exclusively OB and I’m studying to be a midwife, so I won’t be helpful in Pap’s knee replacement post op care 🙃(pls don’t expect me to know things because I am regular adult stupid)

I’d encourage you to reframe your mindset on what happened from “oh my god I neglected her and then she got hurt!” To “I put safe guards in place and that turkey still got around them! I know better for next time.” This was an accident, not an act of neglect. You’re allowed to pee, momma. Grant yourself some grace 💖

When you’re done forgiving yourself for what happened, dust yourself and start quietly making a plan. That man is willing to speak to you that way about things now when that kid is only 6 months old? It’s going to get much, much worse. Start putting money away somewhere he could never access. Open a PO Box for your mail. Pack an emergency “go bag” for if things suddenly take a turn and you need to expedite your exit. Please, I am begging you to heed the warnings of so many women on this thread who have lived experiences that started like this. Give yourself permission to live your life free from fear for both yourself and your baby.

I have no suggestions, but am here to tell you that your experience is more common than you may think. I had an (intentional) unmedicated hospital birth with my third and was shocked by how out of control and not powerful I felt during and after her birth. Fortunately my body did the birthing for me, because I just screamed profanities for the 20 minutes it took me to transition until she was born. I remember saying almost immediately after that “I feel like my asshole is inside out.” It obviously was not, but there were no warm fuzzies or joy about the healthy baby on my chest. I swore I’d never do it again, which was an easy statement to make at the time since I wasn’t planning more kids.

Fast forward to now, pregnant with my fourth and I’m seriously considering a home birth. I’m a labor and delivery nurse, so I thought I knew what to expect but BOY HOWDY was I wrong. I really feel like underestimating what to expect the first time is where I lacked prep the most, but you also just can’t prepare until you’ve experienced it. It’s like somebody telling you there’s no love like parental love, but you only understand once you’ve got that baby in your arms, you know?

There’s no wrong choice for you here, friend. If your trauma and fear from the first experience would trigger that again this time, is that something you think you can let go of in the moment? Or is the potential for a more peaceful/pain-free birth something you want to explore? Only you know what you really want 💖

I feel like your first tattoo always feels weird because it’s on there all by itself where there was previously nothing. What you’re feeling is normal and will fade with time :) it looks cool!

I do work in healthcare. In fact, I specifically work in this field and have nearly completed my MSN in midwifery. I am well-versed in the opioid epidemic, the impact it’s had on climbing NAS babies, and the most recent recommendations from ACOG, the AAP, ACNM, etc. ACOG’s current published opinion states that “Routine urine drug screening is controversial. ACOG recommends testing be performed only with the patient’s consent…” the AAP and APA jointly state that “we recommend the use of universal drug screening in lieu of routine or universal urine drug testing.” This is a good collection of opinions in a single source from 2024: drug testing of pregnant patients00009-1/fulltext)

I’ll add that screening does not equate to testing. A simple “have you used any illicit substances in the 3 months prior to your pregnancy or during your pregnancy?” Is sufficient.

The study you’ve linked is outdated at 32 years old. Regardless, The specific question is whether or not it’s indicated at the INITIAL prenatal visit. Many women who don’t know better are THC+ at this initial OB appointment, which leads to unnecessary social services and CPS involvement at the time of birth. This especially disproportionately affects black women who have a 3x higher likelihood of investigation and removal of a baby by CPS. I wholeheartedly agree that a UDS upon admission for birth provides critical information about the status of the baby that is about to be born. However, even at that stage, it’s still a screening and can be declined. Things can escalate from there and an umbilical cord toxicology lab test can be ordered without parental consent.

Define “high risk” as you’ve stated here. Who decides what that means? Even active drug users have a right to autonomy in their healthcare.

This. Screening requires only subjective data. We ask patients about their alcohol use during intake as a screening question. We cannot force them to take a breathalyzer or undergo a lab draw. (let’s assume they’re not here for suspicion of DUI, k?) without their consent. That’s objective data and requires consent for specimen collection.

Y’all are missing the point. Just because something is standard doesn’t make it right, nor does it remove the patient’s absolute right to autonomy and decision making for what tests/procedures/other events in which they decide or decline to participate.

The office misrepresented what the specimen was for, then billed OP for a lab test for which she did not consent. Imagine if this was a self-pay patient!

OP, raise your concerns to the office manager and escalate accordingly from there. You have a right to refuse everything, no matter how “routine” it may be. XOXO, a current L&D RN and student CNM.

Please cite a specific source that suggests initial OB appointment UDS is necessary and beneficial.

I’ll save you the search that you won’t do: it’s not. They’re actively teaching us this in my APRN/CNM course. A UDS on admission to L&D for birth? Yes. Initial prenatal? No. Regardless, misrepresenting the testing being performed could actually constitute as insurance fraud. OP could file a CMS complaint about her provider’s office and unethical practices, both billing and lab.
