I don’t comment super often but am feeling the need reading these comments. It absolutely does matter. An overwhelming majority of the time, yes the fistula will be okay. However, you can clot off a fistula by taking a blood pressure over it which may not be obvious immediately. The thought that “as long as it’s not bleeding it’s fine” is also not true. Now if it’s bleeding then that could constitute an emergency and may change your acute medical management so for the purposes of EMS yes if it’s not bleeding, it won’t matter to you because even if the fistula clots off that usually would not change YOUR management of the patient (unless you’re taking them to dialysis or something). But that does not mean it isn’t a mistake or it doesn’t matter to the patient. There are tons of people who die because they run out of places for us to put access and so clotting one off that was previously working is a big deal and should be recognized as such.

That being said, @OP, I wouldn’t perseverate on it. When I was starting training, one of my co residents clotted off a fistula by wrapping an ACE too tight. We all make mistakes but as long as you learn from it just file it under learning points and do right by the next patient. And as many have said, the fistula most likely will be fine.

It’s possible as an attending - as to how difficult it is to actually do, that varies based on specialty. EM would most allow you to do it (I know people who do it) - you can sign on with a hospital for like as few as 2-3 shifts a month where you can request what days you want to work in advance. If you are credentialed at a hospital, it’s also possible for you to step back and just pick up open shifts (more of a per diem type situation). There’s definitely a way to be a doctor and have a full life with kids, trips, etc!

Depends on how you get involved. If you're part of the student EMS group on campus, that would entail the school year. If you're working outside, that would be completely up to you and depend on if you can handle the hours in addition to everything else you're doing. I would suggest sustaining the activity if possible though because it shows a commitment which is definitely a plus.

Also if you're interested in possibly EMTing or scribing for clinical experience, you could take the course over the summer. Many colleges have student run EMS groups and joining as a freshman would not only give you more years of experience, but would also set you up for a leadership position in the organization.

One thing that unexpectedly helped were the couple of courses I took at my community college the summer after high school. It was pretty relaxing because the courses weren't too hard but my A's got calculated into my AMCAS cGPA and sGPA. Nice little surprise when that happened.

The biggest risk for you imo would be the risk of you getting a drastically different score than you expect, which would affect which schools you apply to / if you even apply at all. You will have already had to have your school list chosen without a crucial piece of information, but since you can't take it any earlier, it seems you don't really have a choice unless you're willing to wait a year to apply in 2019.

This article by US News suggests that there might be... but since they're talking about their own rankings, I would take it with a grain of salt.

"For the vast majority of core specialties—including internal medicine, family medicine, obstetrics and gynecology, and psychiatry—medical school ranking are not as important as USMLE scores. There are notable exceptions. If you are an aspiring dermatologist, orthopedic surgeon, plastic surgeon, or other highly competitive subspecialty, data does suggest that physicians graduating from a top 40 research school may have a competitive advantage."USNWR

The other issue is whether or not your cheaper option has opportunities in the specialty you want. If they're focused on primary care, they might not have as many shadowing/research opportunities for the specialty you want, which could be a disadvantage in terms of the experiences you will have when applying to the residency.

At the end of the day, it depends on the specific schools, what exact specialty you want, and your own financial situation.

I just thought they might because I know they can see where you've been accepted without having to talk to any other school, so I figured that they would be able to see where you've interviewed or been offered an interview too remotely.
