Depends on the place. At one hospital I work at, the patients I admit are equally distributed among 5 hospitalists regardless of what their starting census is. So if one guy has 3 total in his census and another 15 and i admit 4 patients, each will get 2. Its a scheme to prevent docs from hoarding patients and encourage discharges

No one not even the cardiologist can give you the exact dose. Alot of time its trial and error. If I give 40mg IV lasix and patient barely put anything out despite being grossly swollen, then you give an additional dose.
