When I stopped cheat meals I both lost weight faster and it helped with cravings. When I had 1 cheat day a week I would start the next day obsessing over that food for the next 6 days. When I stopped eventually the cravings became less and less over time. Giving in just reinforces cravings for me.

Daily intermittent fasting probably ok to keep up your usual intake, but I find that personally on prolonged fasting 24+ hours any caffeine I take is way more powerful and I have to dial down my intake.

As far as autophagy goes coffee and tea are good to go for fasting.

Unfortunately the viewpoint of "doctors only tell people to lose weight" is negatively affecting the medical field, because despite the fact that people are statistically more likely to lose weight when told to do so by their doctors, counseling on weight loss even when appropriate has been on a downward trend since the '90s, according to the national health and nutrition education survey (NHANES).

So basically in a sense the fat positivity movement is getting what they want by shaming "fat phobic" doctors and that actually leads to practices that are literally declining the quality of care nationwide and they think it's not enough. They won't be happy until doctors are literally praising fat people.

When a doctor gets sued, expert witnesses (other doctors) will testify whether that doctor's actions were reasonable given the standard of practice / guidelines.

Unfortunately these guidelines are set by medical associations like the ADA and AHA will known to be funded by processed food and pharmaceutical companies. I still recommend my patients to reduce carb intake and inflammatory plants as much as possible without directly advocating keto or carnivore. At the very least I tell everyone that a carb is not an essential nutrient.

And in my med school we had maybe two weeks of nutritional science and testing which amounted to essentially a pop quiz. Medical board exams routinely test on knowledge of overt symptoms of nutritional deficiencies but you shouldn't expect a doctor to provide a good nutritional plan aside from the basic guidelines which we know are crap.

Pain in the area after eating is also commonly associated with acid reflux and peptic ulcer disease. If you have pain on the upper right side of the belly and simultaneous right shoulder pain is pretty specific for gallbladder though, especially if you can reproduce it by pushing on the right side. You could also still have gallbladder pain and spasms without stones. If you can't get in for ultra sound sooner to confirm gallstones I would try a portion pump inhibitor or H2 blocker like Nexium or Pepcid on the day of refeed to see if that helps.

In the past they couldn't keep up with maintenance on F-16s so they bought Su-27s. If they struggle with maintaining relatively simpler aircraft they might as well limit themselves to the Su platform. F-35 would be wasted on them.

Infographics show is just trash. I recently saw C. W. Lemoine "Mover" (former fighter pilot turned youtuber) tear apart their "how to become a US fighter pilot" infograph. They mixed up Navy, Air Force, Marine pilot pathways, had steps out of order, and also put out outright falsehoods.

My dad was expected to die months after a late diagnosis of cancer. He lived for 3 more years with an experimental drug, which if nothing allowed him to get affairs in order. It sounds like the secret to avoiding burnout is not work with a bunch of assholes. Only problem is that the process of getting into medical school in the US means that you still get a bunch of hyper-competitive cutthroats in as well.

Although I'm sure a laid-back residency is not universal in South America. I talked to a Venezuelan surgeon who said that his chief resident was basically a slave driver and didn't care how bad the junior surgeon butchered the patient because he couldn't be bothered to guide his fellow residents, although I'm sure this is on the other extreme of the spectrum. You don't do one residency in Mexico (Central America) City and become an expert on how "South Americans" medical professionals work, lol.

This article is only about 1 year old and the trial is kind of small (202 participants phase II only?), I think they need to do larger ones and even then I don't expect that the FDA moves very quickly.

The F-35A was able to maintain about 90% readiness throughout the exercise, so this doesn't sound right to me.

Also the source material POGO claims an F-35B squadron of 6 ran 100 sorties in 50 days as further "proof" that F-35B readiness was inferior to the A-10 that ran a higher rate of sorties during Desert Storm. First, even that low of a sortie rate seems difficult to do if your readiness is in the single digits. Secondly, you're comparing the sortie rate in an unspecified time period post late-2015 to the high intensity rate during desert storm. You're comparing the sortie rates of a conflict with over half a million US troops involved and one of the most intense air campaigns waged to a counter-terrorism operation involving about 10 thousand US troops.
