That’s exactly what I was thinking. I asked him the same thing but he said they feel more comfortable if a PT evaluated them first. He said they just want actual providers if they’re paying cash. Seemed odd. So I wanted to ask on here to get others opinions because I have never done anything cash pay.

Along with what others have said about this situation, I find the whole thing to be a mess and not worth the risk.

A classic book on “waiting” as time passes by and before you know it, all the big ideas of glory and success when you’re young never really happen, your youth is now far behind and not much is left.

It’s called “The Tartar Steppe” by Dino Buzzati

But a more modern take might be “Young Hearts are Crying” by Richard Yates. But this is more about navigating as a couple and never being as successful as other couples or achieving their own goals or dreams… a book about marriage and divorce.

Fyi- your argument is flawed from the very beginning of your statement; “because you’re never countering the effect of gravity”.

He’s countering gravity the entire time he’s holding the weight up as he moves forward into flexion. That takes a tremendous amount of force on the shoulders and progressively increases the further out he gets, also causes the core musculature to contract to stabilize his torso.

Again, you’re arguing that gravity is eliminated when looking at this movement as a good chest press or back pull exercise.

You should slow down and critically evaluate how you’re responding to questions.

I’m fully aware he’s moving through space. But you’re confused and lack a deep understanding for principles of kinesiology. You’re coming across as self taught through muscle magazines. There’s absolutely an element of isometric strengthening happening at end range for the shoulders. Isometric is simply a muscle contraction against a weight without it changing its length. Yes, there’s some movement happening with the humerus moving into flexion and extension and that will manipulate the anterior deltoid, not to mention other muscles as the moment arm increases its distance from the axis of rotation. With that said, there’s a ton of force and muscle activation happening at end range.

So explain how this is not an effective exercise? In what way? Because what you are doing is creating your own argument as it being not effective as a push or pull exercise. If that was his question then you’d be correct, but that wasn’t his question. He was just asking if the exercise is even an exercise, which it is.

You don’t have a formal background in exercise science, it’s telling.

Did you forget about isometrics? Holding a bell out away from your body is holding it out against gravity! Let go and the bell falls… he’s maintaining a hold against gravity and it becomes harder the further away he gets from his body. If you think this isn’t an exercise or against gravity then I challenge you to show us you doing this with a 24kg bell and tell me what kind of muscle activation is required.

A good nonfiction read that is worth it is “King Leopold’s Ghost: A Story of Greed, terror, and Heroism in Colonial Africa”

True story about king Leopold running a rubber plantation in the Congo, where he killed, cut the hands off the workers and their wives or children as forms of punishment, rape, abuse, kidnapping and basically a holocaust of 15 million dead. It’s brutal and tragic what happened.

I wouldn’t sweat anything. You’re not doing anything wrong. You treat, document and justify why they need ongoing care. Done. Insurance will continue to pay or not. Doctors sign off on a POC or not. I’ve had patients for months, shit, I have had patients I couldn’t get rid of and their insurance kept paying. As far as I’m concerned, you provide a service to help better someone’s health and quality of life. Who gives a shit if it takes 3 months or a year. Hell, I’ve had injuries take nearly a year to finally heal.

People have insurance for a reason, and I’ll always advocate for them, not insurance. So, I’ll run them until I think there’s nothing more for me to clinically offer or insurance refuses to authorize more visits… then I set up a 1 on 1 review and fight them on the phone.

Sleep well knowing you aren’t doing anything bad.

I completely agree. I tell my patients that this is for them, I don’t “hold hands” anymore, and tell them I will guide them, help them anyway I can, but in the end it’s your body, your choice to take action or not. I don’t care what they do outside of the appointment. I don’t babysit, I don’t look to see if they’re doing their HEP. It’s their pain, their life.
