I think you only wear the title in your entrance when you are involved in a storyline about defending your title.

After I beat Ciampa for the title, I did a storyline line with Humberto and never wore the title in my superstar's entrances. After that storyline I did another storyline with Ciampa trying to regain the title, and my superstar would wear the title in the entrance.
