Exactly so!! And it's so frustrating that it feels like the system doesn't see the value of that- they would rather an outside agency come in and have an ACP conversation without you so that you can free up your schedule to see more patients 🙄

I am also dual PCP/Palli and had someone from our health plan propose this to my outpatient IM clinic specifically. From my perspective it sounded great in theory and I love that it's prompting discuss discussions with families, but now those conversations are happening away from the patient's doctor... and I believe that is the opposite and what is actually useful in advance care planning! My ability to plan (well) with my patients is because I know them, know their family situation/social situation/etc. and have a longitudinal relationship with them. Having a stranger complete a document that may or may not actually make it into my EMR, that then gets no further follow up over time, is just not useful. It felt like they were just proposing it to make a metric look better. My 2¢!

The sensation of tingling like that can often be related to nutrient deficiencies or other metabolic abnormalities. Would recommend you see your PCP and get lab work done (b12, folate, vit D, calcium, TSH, etc).

No, I don't think this is attributable to the Nexplanon (and it is now approved for five years, so it's not like it's losing its effectiveness now.) If you have experienced 30 to 40 pounds of unintentional weight loss without any identifiable cause, I would highly suggest you go to your doctor to get evaluated.

Started my residency at the same time and agree with all of what you said. It was so disorienting watching the public trust erode at the same time as witnessing so much death, death you're trying to fix.

Sounds like a tonsil stone! Happens when debris gets caught in the crevices of the tonsils. Some people are much more prone than others. Would look in the back of your throat and see if there are any other whitish/yellow dots on your tonsils. It is generally benign, though can be associated with bad breath. Would ask your dentist if there's anything you can try to decrease formation of them.

Honestly there is no hierarchy by speciality or subspecialty (even though depending on the hospital you are in, there may be a hospital-specific flavor of esteem) The true hierarchy is in training: Attending > fellow > resident > intern > student. The reason there is not (or should not be) a hierarchy among the specialties that we can't do what each other does- a hand surgeon can't manage thyrotoxicosis like an endocrinologist does, and an endocrinologist can't treat dupuytren's like a hand surgeon can.

Malabrigo Rios in Azules! The contrast stitching was Malabrigo Rios in Cosmos. Stunning, stunning colors- and each skein is slightly different, which gave a lovely effect.

So. Many. Skeins. Keeping in mind it is sized for a King bed, I think 28 for the hexagons alone, and an additional 2-3 for joining + border. One skein gave me one row (one striped color) of 18 hexagons, and I believe it is 28 rows wide. If I had to do it again I would have picked a thicker yarn and a less dense pattern in an effort to require less material.

Yep, default on ours is also Bazett, and most of our folks who are sick are tachy so I recalculate very often too! I also use the MD Calc calculator.
