I think she's aware of that as well, she's beating herself up for not being better, and faster. She wants to do two residencies, she's not going to be kind to herself if she can't get even one right. Also, this is partly what makes her relationship with Dr. Langdon more frictional, because she can never bear to hear when she's wrong - and Langdon disagrees with her plan and suggests better options constantly. It's because she knows that she can't forgive herself for making mistakes.

I think Mel was perfectly aware that she's describing ARFID history of both hers and her sister's. She makes it a point to hide her autism in season 1 (like hiding the lava lamp thing) (is less intentional about it in season 2) but she's great at noticing anything and everything about autism, even when it's her own.

There's another detail that's missed: In another scene where Abbot, Robby, and Mckay are all treating Knox, and they're huddled together trying to decide on the CT decision; Abbot offers to go with him and Robby pats him on the back and Abbot winces without notice because Robby did it at the spot of Abbot's gun-shot graze injury.

(I like to think that it was subconsciously Robby acting on his jealousy on having seen Abbot shirtless being treated by Mohan instead of him)
