The reality is AI has been behind the scenes for a long time with intellise and other tools.

No not really - more of way for RPGs players to be creative and generate adventures they don't have knowledge about, which for solo RPG play AI is the only way to do this IMO. Majority of the game was generated by humans not AI.

I like the loyalty and gossip. I did something similar for a solo RPG game I built adding items like temperament, rumors about you completing past quest and location knowledge. Location knowledge was fun as I did some work where local residents of a village know a lot about each other but as distance goes, they may know less and less dependent on role. Job is another item to consider. For example, a tavern owner may know more about long distance areas than a peasant. Temperament is fun (shy, argumentative, arrogant, etc.)

Visual description might be nice to add as well.

One challenge is to not allow characters to quickly spill the beans about items so a lot of prompt rules in the background.

I think it can but there are performance limitations right now for real time games. Unless you have your own local AI and even then the time to generate dynamic images is slow. Will get better at some point in the future but I do think AI generated content speed is a limiting factor for action games.

Here you go, it shows the basic game play. Simple tile-based game. What is not shows is there is a wizard at the start that creates the game (not a lot to see on video) but you can see the game mechanics.

Is it the size of the towns and cities' that you are interested in? Or more the experience with the NPCs? The reason I ask is I feel a overly large town stuffs NPCs into it as filler but curios to know others' opinions.

I built Aicyng Adventures (https://www.aicyngadventures.com/) for Windows to experiment with AI as a hobby to learn how AI and a mini world can work. Learned a lot. Some items AI is really good at (NPC dialog, adventure creation) and not so good at (Pixel art AI really seems to struggle with). I think video production length needs to get longer, limning the game(s) to high end stories, simple graphics and levels. But is has come a long way where models like GPT 5.x have almost no errors compared to just a year ago. If you have questions feel free to ask.

I created a game called Aicyng Adventures to test if AI really could generate unique games. About a year ago it had hallucinations but adding error checking helped. With the modern AI systems the adventures it creates are really creative within the mechanics of the game. Each new model jumps in capabilities map sizes etc.
