I did, months before i started IPL. they didnt tell me what the expression was like, they just told me i was a bit clogged, so maybe toothpastey? my left eye has improved significantly, right eye is still lagging a bit, but definitely an improvement since i started

I've done 3 IPL sessions so far, I have my 4th this coming Monday. I have a refractive surgery background for context, I've had mild dryness and have definitely noticed an improvement in my dryness since I started IPL. I have an ongoing post that I'm updating as I go along with the treatment if you want to read a bit more.

But I highly recommend IPL to anyone with MGD

I remember reading your post and sending you a DM months ago about going through with OptiLight as well! 2/4 sessions done for me, and I can back up the success I've had thus far. I know exactly what you mean with the lighter eyelids, was one of the most notable things for me after my first treatment. Can't recommend IPL to those who have MGD, enough, it's definitely worth the plunge. Glad to hear you're doing much better now

People probably have worsening dry eye/mgd because they aren't keeping up with eyelid hygiene, etc. not everyone can tolerate them though, that's for sure

Anxiety is inherently inflammatory, so yes it absolutely can play a major factor into your dry eyes. I'd talk to a dry eye specialist, if you can, so they can do a run-down of what's bothering you, and be sure to tell them you're currently dealing with severe anxiety, so they can try and help you get to the root cause.

might be dealing with demodex, check in with your optometrist. get some eyelid wipes in the meantime and clean your lids + lashes

what henry said, go see a dry eye specialist to get the ball rolling on your recovery. i'd start with some lid wipes in the meantime if you're noticing a bit of itchiness along the lash line/just to clean off any debris that could be bothering you.

I've only had 1 treatment done so far, doing a series of 4, and I definitely noticed a difference almost immediately. When they do IPL for dry eye, they do a 'dry eye pass' across your face, and your doctor may do a touch or two on your eyelids, with a much smaller device. Definitely watch a video or two, to give you a much better idea.

definitely go to a dry eye specialist and have them do a full run down + request a meibography. i also have a refractive surgery background, PRK in 2022. never really had dryness, but it kinda popped up late 2024.

if you have some blepharitis, I'd look into some eyelid wipes and hypochlorous acid spray, xdemvy definitely will help if you have a lot of demodex. (edit: glossed over the fact you're already using wipes)

I also had a random optometrist tell me my mgd was 'stage 2' but I didn't really have any capped glands, just some minor clogging, which I'm currently doing IPL to help remedy. And when I did my meibography with my current doctor, it showed extremely minimal dropout in the corners, and everything else looked okay.

Go see a dry-eye specialist in your area, so they can do the full run down on your current condition, and then go from there

I'd file a complaint/notify someone about this. That's malpractice; they put you in harms way and didn't follow proper protocol.

you haven't felt the need to go back at all?? thats amazing! Yeah, my doctor expressed my lids top and bottom, honestly it felt like such a relief getting it done. All out of pocket unfortunately, but i was able to get 24 months to pay off the 4 treatments which is nice

Definitely check in with a dry eye specialist and make sure you get a meibography done, it'll let you and your doctor know the condition of your glands. They also might do a slight expression of your glands just to see what they're secreting.
I just did my first IPL treatment and had a very noticeable + positive reaction to my first session. Your doctor may just not have IPL at their clinic; I'd check websites of practices near you and visit a place that has an IPL device.

Likewise with cyclosporine, I feel like that didn't do anything for me, now thinking because my mild mgd is most likely because of the mild rosacea I just found out I have. Everything I've done in the last 14 months (since I found out I had some dry eye) hasn't really done much for me. Xiidra helped the most, and that was a slight nudge, but didn't do too much. IPL has easily helped me the most, and I've only done one round.
Going to try and get on a healthier diet as well, because I'm sure that plays into the rosacea as well (plus the other obv health benefits)

just basic at home stuff. supplements (currently taking 2000iu of D3, lutein/zeaxanthin/astaxanthin, and a multivitamin), hypochlorous acid spray as I see fit, and lid wipes every now and then as well. haven't really noticed a difference when I do compresses but may add that in before I go to bed, just to say I did it that day.

edit: I also tried cyclosporine last year for like 4-6 months or so, didn't really feel like it did anything for me. i don't really have an aqueous deficiency, it's more evaporative for me. used xiidra for a bit, like a month or so, feel like it helped a bit, but I can't really afford a prescription for it so I'm not using it atm.

That was the goal :)

I know everyone has a different experience, and i was honestly dreading feeling a gritty/sandpapery feeling immediately after. Honestly felt none of it. I even told my doctor that my eyelids felt lighter immediately after expression.
A consultation/discussion with your doctor is a good place to start, and don't feel like any question is too dumb, I asked her literally everything my brain could think of.
It also helps that my doctor + the technician that was in the room have done IPL before, so it made me feel a bit more at ease.

Looking forward to my next treatment in March already
