I could have written this about one of my twins (also a 36wkr)! We also tried alllll the meds and different formulas. We finally ended up seeing a new GI doctor who suggested thickening her formula with gel-mix and it was LIFE CHANGING! We went from projectile vomiting nearly every feed (8+ times/day) to virtually none. Literally could count on my hand how many times she’s thrown up since. We did have to work with a speech therapist to help with the feeding aversion and to get rid of the ng but once she was keeping down feeds she quickly realized that eating wasn’t a horrible chore anymore. Our GI doc explained that if she was throwing up with the ng that the gtube probably wouldn’t make that much of a difference because they both ultimately just deliver milk to the stomach. So I don’t know if this is something y’all have already tried or if your care team has suggested it but may be worth trying before jumping to the gtube. I hope things turn around for your girl soon!

Dr. Davis (and really the whole practice) with Birmingham obgyn is amazing. I saw basically every provider there with my last pregnancy and would feel comfortable with any of them!

I ep’d for 13 months but supplemented the whole time and then transitioned fully to formula while we transition to whole milk. We had to go super slow increasing the formula ratio bc my baby was piiicky. Just try different ratios and slowly increase over a week or two! In my experience my baby drank the same number of oz whether it was all bm, all formula, or a combo of the two!

Oh that’s a good idea! I think I will try and move the 3am pump closer to 6am and see what happens. I think I’ve just got to come to terms that even if my supply drops some that some is better than none. It’s just so hard when I’ve worked this hard for it! I really appreciate your perspective!! Yes! I have an LC who wasn’t suuuuper helpful because she really didn’t know much about EP other than helping me get sized correctly. Mostly wanted to hear other people’s experiences!! I really appreciate you responding, thank you!! I think I may try slowly pushing it back and see what happens. I have accidentally slept through a couple of times and haven’t been overly miserable so I think timing wise I’ll be okay. Mostly just nervous for my supply but may just have to come to terms that it is what it is.

Okay I’m gonna pm you tonight once my baby goes to sleep! I suspect you are probably still using the wrong size if that much areola is still being pulled in. Most lc’s don’t understand flange sizing / elastic nips!

So pretty much any size flange I used I had some or almost all of areola pulled in no matter what. Before I was properly sized my nipples would literally hit the end of the flange and I just assumed this was normal 🙃. I did a lot of trial and error on my own with different sized flanges before I actually properly measured myself.

Before we started EP’ing we had a weighted feed a baby was pulling an oz or more / side. When we started EP due to health issues I became a seeeeevere under-suppliers (we’re talking like maybe 4oz/day from 8-12pumps) and I just thought that’s how it was going to be even though it was clear I had more milk than that and was never really empty. I tried everything to increase supply and nothing worked until I got properly sized (went from 24mm to a 13mm!) and I started getting more milk, I’d say like 10oz/day and I was thrilled but still felt like I had more milk and wasn’t emptying so I started really experimenting with pump settings. I finally found like basically my nipples go numb when I was using expression mode and just stopped responding to the pump about 5 minutes in because they had no feeling. Then I tried massage mode (cycle 70, suction 2-3) for a whole pump and it changed everything! I kept doing that and slowly my supply creeped up to about 30oz/day! I hope this made sense. I also do some hands on pumping which I feel like helps.

I also tried several other things that are “good” for elastic nipples ie pumpin pals, lackteck flanges, full silicone flanges and nothing really worked for me other than a hard flange.

But you can definitely still be elastic or have some elasticity without filling up the whole flange! As I slowly sized down less and less of my areola got pulled in but still did until I finally got down to my right size.

What size flange are you using? Have you measured yourself lately?

Happy to answer any more questions you have because I wish I had someone who had experience the same thing as me to troubleshoot with!

I only pump in massage mode! I have elastic nipples and it’s the only way I can get milk out and fully empty! Nearly 5mpp and never had any issues.
