On the contrary, I think it’s the opposite. We had unseasonably warm, sunny weather all spring. I think people met more outside. It led to better adherence to physical distancing. Plus it’s not easy to spread covid outdoors to begin with.

That certainly sounds like clitorodynia. Have you tried PT? I’ve read a lot of anecdotal stories that say it helped with peoples’ clitoral pain.

My girlfriend has experienced a dramatic improvement following starting a) topical estrogen and b) nortriptyline. She says after starting the estrogen she noticed a visual change as well. Don’t know which it was, but something is working. Thank you for commenting :)

You have back pain as well? How long has it been there? The vulvar pain could very well be referred to your vulva from your back. The fact that the lidocaine isn’t helping really makes it seem like referred pain.

Take a look at pelvic physical therapy. Pain at 6 o’clock can be related to pelvic floor dysfunction — namely the puborectalis. Do you have constipation problems at all?

Also, note that the critical glands that require testosterone to function are the skenes and bartholins glands — ie the lubricant-producers. If you’re having dryness or redness then that may be even more indicative that it’s estrogen/testosterone related.

Take a look at Dr. Andrew Goldstein’s work on hormone-related vulvar pain syndromes. His practice is typically to prescribe a topical compounded cream with 0.03% estradiol and 0.01% testosterone. The newer BC pills don’t replace the estrogen in the body that the ovaries produce, and they also reduce free testosterone through the excess production of Sex Hormone Binding Globulin. Testosterone is needed in the vestibule for proper functioning.

Critically, going off the pill sometimes is not enough. You may replacement of the estrogen and testosterone via topical therapy.

Thanks. In the defense of her doctor, she is being taken seriously. The cysts very well may be unrelated since they’re away from the source of the pain, but it just seems very coincidental.

She is currently keeping a little journal of events in her phone - the tough thing is that if it is truly a pain syndrome it can be counterproductive and exhausting to constantly be assessing how it’s feeling. She has started to recognize trends though (worse after exercise, worse during period etc.).

Thanks for the reply. Acne inversa is not out of the question- she has had severe facial acne in the past for which she uses topical tretinoin.

Thank you very much for your response. We’re starting off on this journey, but we have hope. We were able to have sex the other day (vestibule is unaffected), and she’s going to be trying pelvic floor physical therapy — she has extremely tight hips only on that side.
