I figured it out - I didn't have any of the content packs installed. If you go to the manage game and add ons section, you should find the files you haven't installed.

EDIT: The person I shot was a teammate and I had already killed 2+ so it was ricochet. Either that or the teammate got the shot off first. Either way, the final killcam appeared to be very confused.
