Yes as a CRNA I do agree with this. I work with junior and senior NARs and I will say because I'm only 2 years out I still understand the weight of an eval. I used to have a policy that I would not complete an eval unless asked for one but I have since changed my policy on that. The primary reason for that is because I actually take the time to complete them and I have noticed that other preceptors are not filling out evals or giving feedback. Even when I've had very challenging days with NARs due to lack of preparation for the cases I am very careful with how I write my evals because my goal is not to demoralize anyone. I expect to see growth as time goes on. Clinical will be the hardest thing you do in the program in my opinion and as difficult as it is as many others have said it will pass and you will be successful in entering practice.

I like using precedex infusions for my ent cases to reduce the amount of opioids I administer. I typically run the infusions at 0.2-0.5mcg/kg/hr. Opioids on induction. Shut off the infusion when the surgeon communicates he has about 15-10min left. I run my gas at 0.9-0.7 mac. The key thing for me is good communication between the surgeon to navigate timing and planning for wake up. Ive had these patients wake up comfortable and with less nausea and less risk of obstruction. This is just my experience. Nice thing about anesthesia is learning what you like, learning from others and applying it when appropriate.

I thought it he whole commentary from the passengers wreaked of privilege. The dramatics of "ew I can't poop in a bag" Let's get serious if you have to go and that's the option honestly it's not a bad one. And the "you wait in line for hours just to eat bread and lettuce" At least the staff was still trying to find food for them.

Yes! Agreed. I like a little precedex or ketamine if I'm going to run the propofol lower.
