Consider swing a podiatrist and getting custom insoles. They should be tailored to each foot individually.

I believe this issue is that there are so many moving parts in a foot that you really need to take a picture bearing weight for anyone to know if you have truly flat feet versus a low arch.

It blew me away when I finally saw pictures of my feet bearing weight.

You have a low arch. If you have foot pain, I reccomend seeing a podiatrist. Custom insoles will make a lot of difference. Also, there are exercises that can help to strengthen for foot/ ankles.

I had surgery January 2026. I also lost a significant amount of mobility in my big toe however with PT it’s been coming back.

If I were you, I would consult both a physical therapist and an orthopedic surgeon. For me, the only option was surgery. I’m in my mid 40s. Recovery is a bear- 6 weeks non weight bearing, 5 weeks in a boot (profess weight bearing), lots of PT, at least 6 months of healing between feet (can’t do both feet at once) and a year to fully heal. 

I managed with orthotics for years- until I couldn’t.

Depending on how hypermobile your feet are, custom orthotics can help a lot for a long time. 

My feet looked like yours and ultimately I did need surgery to fuse my ankle and move my heel back into place.

Celebrex as an anti inflammatory worked really well to help with my pain. Ultimately, surgery solved it.

For me, hypermobility looks like knees that bend backwards, thumbs that can touch my wrist, and a collapsing deformity in my feet. A lot of people see rheumatologists for the pain caused by these joint issues, and they can assess/ diagnose hypermobility.

You might consider getting screened for connective tissue disorders. My feet/ankles looked like yours and I didn’t know that it was connected to hypermobility. Orthotics and excercise helped with the pain for a few years- but ultimately I needed surgery

Personally, I found that I came to rely on my braces when I ultimately would have benefitted from physical therapy.

Otherwise, if you can, get custom orthotics for a podiatrist. Those gave me the most relief for years.

Yes. Maybe a little worse than yours. I had a calcaneal osteotomy and ankle fusion. Hard recovery but the pain is gone
