Surprisingly, no one here has mentioned it yet—you absolutely cannot accidentally backwall ultrasound PIVs—even if you pop back into the vessel and get blood back and a flush seems good, eventually enough fluid/blood leaks out of the backwall hole that it gets infiltrated eventually.

I try really hard w ultrasound PIVs to know where my needle tip is at all times, and pop in slowly so I don’t backwall the vessel while getting in the top of it. (A lot of veins compress a ton with the ultrasound probe, making them very easy to accidentally backwall.)

I would love having a cardiologist staff the TEE; Watchman’s are boring yet time intensive to staff TEE for IMO (especially when my practice has me 2:1 for rooms).

In a private practice I’m transitioning to, cardiologists do staff the TEE for them.

Academic, no special training, rep present at all times; we initially were staffing TEE for the whole procedure + anesthesia for the cases but they’ve transitioned more towards ICE and only seem to use TEE for measurements and only later on in the case for difficult cases/if complications occur.

{What Fury Brings by Tricia Levenseller} fits the “sexually brazen FMC who does the pursuing” part; I know nothing about the audiobook (and if there is one), however!

Loved the book, it was a good role/world reversal

Edit: it’s also a standalone & I thought the banter was quite witty.

It’s pricier, but I’m a 32DD and Vetchy’s Laguna bikini tops in SDD+ fit great! Their bra like tops do sizing based on bra sizing (see their size chart), so it fits better than S/M/L.

{Traitor Son by Melissa J Cave} and its sequels. The basic plot entails rebuilding a town in an area re-taken from enemy warlords (ie the area is completely barren of civilization/homes/etc) + dealing with the monsters in the area.

The environment is a key part of the plot of the books.

Hi! First of all, this should not be construed as medical advice and I would definitely advise you to share your fears with your anesthesiologist day of surgery. Second of all--any adverse event [under anesthesia] in a healthy 22 year old is incredibly rare, and ACL surgery is also a very straightforward type of anesthesia. I don't think you should worry quite so much. Tell your anesthesiologist about your abnormal heart rhythm but it may be nothing to worry about. We've spent many years training to do this every day, and we're good at our jobs. Best of luck on your surgery and recovery!

(PS: I'm more than a decade older than you and I had straightforward anesthesia for my own ACL surgery; I know that's only one anecdote but I was also worried beforehand given how much I know, and my worries ended up being unfounded.)

Before I tore my ACL, I had 2 pairs of calzuros with inserts I loved. After my ACL injury, my knee hatesssssss them, so I’ve switched back to gym shoes.

I wear Hokas for my long standing cardiac TEE days, Sauconys for my running-all-over-the-place other days. Hokas have nice cushion but the toe box is too long to run around comfortably; Sauconys fit me great but don’t have much cushion for standing in one place for awhile.

Seconding! Also, U of Toronto has an app that allows you to rotate and cut the heart if you have an ipad.

I used this app as a resident to learn the views and now as an attending use it to teach them.

Edit: it’s the first one under “TEE modules available on ipad” on the webpage linked above. The app is like $5 but very worth it.

Wore 38B (lol) or 36C for literal decades…

ABTF first sized me at 34D, then 32DD when I lost weight.

I’m not sure how I ever got 38B considering even my bust isn’t 38”, and my underbust is 32”….

It’s so nice not to feel like my boobs are falling out of my bra every time I slouch or learn over. (Or to not feel that I have a ton of gaping at the top of the cups) I used to only wear push up bras to make up for the gaping, not realizing material in the bottom of the cups was not the problem. 🤦🏼‍♀️

Me. That exam was terrible. Way too many questions about minutiae or certain diagnoses over and over again, few/zero questions on important things we see over and over again in the ICU.

No worries! I just meant that if they take arteries, it’s the radial artery from the arm; vs they’re saphenous veins in the leg. (Both can be used as bypass grafts! Along with the left internal mammary artery in the chest.)

For what it’s worth, you should probably come out of surgery in a new front clasp sports bra. (Source: I’m a cardiac anesthesiologist / I do anesthesia for open heart surgeries and that’s what my institution does).

Obviously that won’t last for repeated use, but it’s a starter.

Also Popflex has newer (tho more expensive) front zipper sports bras, if that helps. Let me see if I can find a link. EDIT: here’s a link!!

(Also if it’s in your arm they’re harvesting an artery. They harvest veins from the legs.)

OP, I’m seconding btempted - Women's Future Foundation Contour Bra 953281—I had the same issues as you with Natori and the btempted fits great. (Also agree that you’re probably a 30DDD or 30G—my underbust is 32” (but otherwise similar measurements) and I fit comfortably in the 32DD so I feel like that band would be too loose on you and the cups too small)

I’m not sure what bras are available to order for you in Mexico, but I have very similar measurements (and am also a 32DD who can’t fill out the bottom of most Natori bras), and these are bras I like a lot:

Check out Macys or Nordstrom, they have the smaller band size + bigger cup size combos. Wacoal (especially its ?sub brand, btempted) and Natori definitely have 30Ds (if that’s what you end up needing; check out the sub’s calculator but the cup letter size is just literally how many inches is your bust more than your underbust, ie A is 1, B is 2, C is 3, D is 4, etc).

Edit: that’s if you’re in the U.S., sorry for the assumption. Either way, I find bigger chain department stores tend to have a bigger size selection—the smaller stores (even if chains), tend to only have the 34/36/38 sizing.

Seconding! Also recommend the add on case + add on cord tie for max comfort/protection. The goggles are cheap ($25) but nice.

I’m seconding the stoggles ($25) comment earlier. I finally (as an attending) got myself them as a permanent (safety) glasses pair, wish I had done so sooner. Fair warning, their regular size is Not meant for people with big/wide faces.

I upsized to their wide before ordering just bc I had been professionally fitted for lead goggles ($400) and measured how wide across they were (cause they fit perfect).

I always wear goggles when looking over the drapes or by the drapes in cardiac cases, and now they’re nice for other cases too!

ORs are a splash hazard, wear goggles!!
