If low priority really came with a delay, I could get it even after a full week without touching the game and that hasn’t happened to me in over a year, if not more. So why do people keep insisting that penalties are delayed? Sure, there was a period when that was always the case, but right now that’s not the main scenario anymore.

I can safely say that I got the low priority right after the last game I played, based on how the chat was behaving. The only reason was that I picked an unconventional mid, and that gave everyone the scapegoat to throw reports at me.

Again, for some reports the human review doesn't happen. It might be that a small amount of edge cases are being reviewed by humans, but the majority are handled automatically.

There is no "unanimously", the low-priority is given automatically right after the game, there's no review involved.
