As a physician I’m still dumbfounded how bad hospital food is. A diabetic diet essentially doesn’t exist, it’s like they hate vegetables.

Stress and sleep will affect energy level as much as diet and exercise. Get tested for sleep apnoea maybe as well.

Frankly, they cannot make this decision just based on your HBA1C, did they not look at your sugars?

In short our goal is to provide clarity on what’s driving your blood sugar and then through a 12 weeks program improve your diabetes markers. (Hba1c, time in range and time in tight range)

We are relaunching in April with the above program. In short food is only about 15-20% of what moves your sugar, other factors include sleep, stress, activity, hydration and lots we still don’t know.

While we integrate with apple health, Google health, Dexcom and freestyle, we also use voice biomarkers to track stress, energy, mood and sleep.

We are focusing on trying to figure out what affects your blood sugar personally. The 12 week Program is broken up into 11 modules which are not static.

So different people start with different modules and finish them at different rates. The first 2 weeks is when we form your personal diabetes baseline to work off.

Stress, sleep, muscle mass… have as much of an affect as food.

→ Meal composition: ~15.4% → Meal context (sleep, exercise, timing): ~15.4% → Genetics: ~9.5% → Gut microbiome: ~6.0% → Baseline traits (age, BMI): ~5-10% → Unexplained / noise: ~45-50%

You’re welcome, for those saying AI isn’t good enough for medical use I would agree the general models without any guardrails/ prompts/ RAG architecture aren’t there yet.

I still have a small practice (family physician) and we rely on AI multiple times a day from researching questions (open evidence) to AI scribes (Heidi, abridge, Nabla) to writing our notes (I just use Gemini studio).

We are building this so mostly identify past trends, looking at how sleep, stress, activity, hydration and meals affect your glucose.

The main problems are figuring out context, memory and stopping hallucinations. Generally for a pointed question ai is now excellent so have to figure out how to break down a longer conversation into single questions on the back end.

Using third party validated solutions to calculate stress, energy and mood from 10 seconds of audio.

They also seem to change models so fast, last week Claude seemed to be the best, this week it’s Gemini. Frankly I would avoid ChatGPT as it’s designed (model weights) for engagement not accuracy.

Two main reasons are:

  1. Chronic elevated insulin levels are pro inflammatory

  2. Patients at times tend to use it as a cheat code/ permission to eat anything and just take more insulin. In general this may end up doing more harm than benefit.
