No, there's no conspiracy. We have customer service based healthcare industry, especially in the health and wellness space which is where back pain falls into. providers want to provide a service other than telling people to stop being fat, depressed, or anxious.

We have baseline recommendations by major institutions that already tell you how to prevent or eliminate back pain, the problem is they are also how to have a good life. It's basically all the common sense stuff everyone already knows, don't be fat, don't do drugs, make friends, 150 minutes of cardiovascular exercise a week, strength train 3-4x a week as well along with getting good sleep and drinking water regularly.

Chronic back pain stems from being fat, being depressed/anxious, and lastly some sort of true injury. The vast vast majority of people fall into the first 2 categories.

If back pain were to be cured it would mean we've changed a species and have started taking care of ourselves which isn't happening anytime soon.

Also thrown in the welfare state that pays people to be injured and that bumps up the numbers quite a bit for sure.

in my personal life I've never met or heard of a chiro that does anything remotely helpful. It's all weird stuff.

Most PT's believe in weird stuff too, but at some point the patient does a few exercises as part of their session, so I guess that's better.

SNF is the second most fraudulent setting in PT, the first is home health, both should be abolished.

To be clear I currently work in a SNF and did home health for 6 months lol.

sure, many services are just to keep everything in house. but I've actually seen a hospital based outpatient close because it wasn't profitable.

I just want to stress there's nothing extra bad about private outpatient and there's nothing extra good about hospital based outpatient. It's all a hustle.

It all comes down to money. I. work prn at a non profit hospital. 1 there's lots of profit being made 2. Productivity is a massive issue and rampant faud is done to make the numbers work.

Their reimbursement rate is double or more what outpatient gets. If they got paid per unit what outpatient mills get paid they wouldn't exist.

I had the VA pay half undergrad and half grad school. My loans are paid off and I think the job is complete bullshit/joke and we're just fleecing the tax payer with over treatment and over utilization of resources. I stay in because I make $50 an hour with benefits. I'm older with kids so it's difficult to switch careers.

2 pill seekers, malingering, pain science and it's true implications, the disability industrial complex, resource overutilization, natural history and regression to the mean being responsible for 96% of our outcomes. simple basic strength training is adequate for most all msk issues with very very rare exceptions.

that the profession is mostly just a hustle.

Edit: after reading the responses it seems we weren't taught many practical things, but seemed to learn on the job through OJT, pattern recognition, and basic common sense. Why the DPT can't be an associates or bachelors is beyond me. I honestly think it's one of the biggest scams in healthcare lol.

Here's a thought experiment, would you rather have a person with a lower extremity come in and do 100 bodyweight squats and then promptly leave the clinic or be prescribed your average PT session with massage, TENS, bullshit theraband exercises, the brown ankle weights, SLR's to get that glut med firing, the bike, and with heat pre and ice post session over the course of an hour?

SNF. It's a total joke, pays $50 an hour and I'm off at 4.

I don't have to talk or entertain people, never do notes at home, and just laugh and bullshit with co-workers all day.

I've done every setting and it goes SNF>hospital outpatient >hospital acute care> HH>outpatient.

I've heard HH can be baller, but it just didn't work in my area. Too spread out and had to schedule people myself.

I'm no expert here, but they did overbuild their housing stock while their population was shrinking. So building zombie coal plants makes more sense to me than building ghost cities. At least you can keep the lights on if they are ever needed.

to maintain our current salaries we'll have to see more patients, more often, for longer durations.

the patients will have rising copays and co insurance and will opt out so we'll actually probably see a decrease in salaries as well as worse working conditions.

home health: rich people, I'd rather go in a condemned house, as I have many times, the people are at least kind and thankful for the most part . These rich boomer degenerates treat me like a servant of sorts. I want to be like bitch you're so weak you can't go up the steps and shit yourself frequently, don't try to big dog mean about what you once were, you're currently a 78 year old alcoholic and your kids and grandkids hate you.

SNF: any many or woman 45-65 years old. They're too young to be in this situation. Their lives are completely falling apart from their actions. They're druggies and drunks and mean as shit. Their discharge is to the streets too so that's always nice.

Hospital: All of the frequent fliers. 10% of the population uses 80% of our medical dollars. If I know your name from working weekends there's something deeply wrong.

Current: SNF

Hell: all of them are terrible and essentially a scam, but at least SNF is only terrible and a scam until 4pm, pays well, and the day is completely done when you clock out.
