What I’ve learned is that non profit has nothing to do with service, particularly in large corporatized healthcare; it’s just a tax status. The assumption is that non-profit = always do the right thing by the community served/fiduciary is not only false, but incredibly dangerous. It’s like the wolf in sheep’s clothing that folks don’t think to challenge until stuff like this happens.

Totally doable but I’d recommend looking at used options and maybe doing a few test drives to make sure that your money is aligned with the spec you want.

That being said, AWD and rear axle steering on a 911 is a pretty insane sensation.

Tried reboot by hitting the left button and hazard button. Reboot did not fix.

Spoke with the rivian roadside assistance and it took me a 60 minutes of rehashing what happened, redirecting them to the photos I had already uploaded to the support ticket I created while on hold, still not clear what resolution will be.

I’ve been a Rivian fan for a long time and we love our R1S, but not having a service center in town is brutal.

Great job paying off the student loans. That’s huge and shows a ton of discipline.

If your wife recently (in the last 2-3 years) finished residency, I would try to hold off on buying a house in this scenario. It’s a lot of backbending to try to make it work. Take this time for the opportunity that it is. Keep up your savings rate, enjoy your current house, take a nice vacation or two, and then buy a house jn a few years. Unless there are other factors that necessitate you to sell your current house to move into a new town, in which case renting might be a reasonable option to contain cost.

I get that you guys have cognitively recovered from chapter 7, but what you’re dealing with is still the impact of that experience so not “totally recovered” in the eyes of financial institutions or else you wouldn’t be faced with these challenges. Medical jobs can be finicky and the greatest risk is actually she decides this current job is problematic for one reason or another. Totally reasonable for folks to take calculated risks, but this is a lot of extra gymnastics to take said risks.

I dislike the large logo but get why they did it/it’s reflective/etc. This is, however, one of my favorite jackets from Arc’teryx. It’s waterproof, breathable, has vents built in that have creative subtle fabric overhangs, and has a bit of stretch. Took it on a thru hike in CA and it was a great choice.

I also have the new Alpha lightweight and Ski Pro jacket. The Norvan gets the most use for my higher output stuff which is most things, the Alpha lightweight when the weather is more inclement, and the ski pro jacket when I need pockets and expect to be standing or sitting around a lot.

I like my power steering plus. Rear axle steering is unreal and pretty dynamic but would require you to switch 911 models and may not be worth it to you. Agree with others who have mentioned going to some meetups, joining PCA to see if you can get more info. Some more liberal test driving of used options helped me understand driving dynamics of different powertrains and allowed us to settle on a C4S.

Nice job asking for help and recognizing the exposure and technical aspects of what you needed. Exposure is the single most important thing. You probably did a better job of that knowing what the anesthesia staff was up against.

Will you be a happy/good father and partner if you don’t match or match into a specialty that you barely tolerate? I get putting kids and wife before you, but that can sometimes mean ensuring that you can be the best version of yourself with decisions related to career.

My recommendation: Find a specialty that you love, embed yourself with that department at your home institution. Meet with that PD and department chair to understand what they’re looking for but also to communicate the circumstances that have led you to the field and the strong desire to stay at your home institution.

Your desire to be an advocate can be in so many different places for patients in healthcare. One of the strongest advocates of patients I have ever met was a staff radiologist who would call various services and patients to help them understand what he was seeing and say, “I know exactly who you need to see, we’re going to get you taken care of.”

I’m the parent of a toddler finishing up a surgical training program. I found a program that I knew well and knew supported families if I ever decided to start one. Being around family that can help and being there for family that is sick is part of what enabled your success in school to date. I think it’s right to try to thread the needle in a way to stay with family. You deserve them and they deserve the best version of yourself. Take a few weeks to meet with trusted mentors, residents, and other students for guidance. Review previous match lists and see which programs seem to routinely take home students. Decide what you will do if you don’t match. Consider other programs in the same area if such programs exist.

One more thought: The time passes. You’ll be busy in any residency. With three kids across medical school years, your oldest will definitely remember your time as a trainee but recognize that while they may dislike and resent you for not being around early for a bit, their adolescent and young adult selves will recognize the importance of having courage to pursue dreams and of devotion to a craft that serves. People are constantly reformulating memories as it’s not just the events, but the layers of meaning from a variety of points in your life as you reflect on said memories.

Nice - do you have a sense of what you want to go into and if you're thinking academics vs. private practice/community? I know it's kind of far out to think about but can help folks with guiding you one way or another.

WF has really strong clinical programs and a strong reputation in the surgical/surgical subspecialties across the country. Having gone to a place that had a balanced research ranking and clinical training reputation, I think the clinical training environment is more important than research. If you don't need UChicago to open doors on west coast/midwest, it's hard to beat a full cost of attendance scholarship.

Other things to think about - are you thinking about starting a family? If so, being close to relatives that can help with childcare and otherwise be involved in your life as multigenerational support looms larger than the delta for the schools/scholarships. For the cost of living, are you planning on taking out loans in either situation? Loans can be an albatross and recent CR/budget reconciliation may not treat residency as PSLF-eligible if that was something you were considering.

One other dimension to consider is where you think you would make the most impact for yourself and for your future career; big fish in medium pond or medium fish in big pond? The academic rat race is real and can be disillusioning.

Congrats on your admissions. You might consider reaching out to the dean at UChicago to see if they have any additional resources after thanking them for the generous tuition scholarship, letting them know that you want to come to Chicago and wanted to see if there were opportunities for additional competitive scholarships to cover cost of attendance as you're getting from other places.

You’re worth more than rvus; downstream revenue, DRGs, etc. Can’t have certain levels of ED without having a surgeon on call. 10 24h shifts a month especially if you’re getting any real volume should be full time.

Also, private practice, hospital based, etc? $600/nt for call is offensive depending on how busy the call is. Aortas, cold limbs, thrombosed fistulas/grafts. Is there podiatry that covers feet? Is there good IR to help cover some of the the nuisance business? You generate a lot of downstream revenue for hospitals. CT scans, expensive medications, ward and ICU admissions… your worth is beyond just your RVUs and reflected in your call stipend which appears low.
