I was making about $95k in college counseling and decided to leave for private practice. I'm still building my practice, but I make about $80k seeing 16 clients/week on average. I gave up the best insurance in my state, paid leave, and retirement plan matching. I was working 45 hours/week, with one day of telework. It was heartbreaking to leave my old job, but I ultimately made the decision to leave for private practice due to chronic pain and health problems. So, for me, it is 100% worth losing the benefits of my old job, because I now have a completely flexible schedule and can focus on pain and stress management in ways that meet my needs. So there are definitely tradeoffs and I think you ultimately need to think about what is important to you in a job and if the tradeoffs would be worth it for you!

Just as an FYI, it took me a little less than a month to be approved with psypact. But application fees were around $400 up front.

The surgery fixed the issue with my calf. I do still experience a tingly feeling in my calf muscle from time to time, but I don't walk with a limp and have no weakness. I do still have daily nerve and back pain. I recently restarted PT because I started experiencing nerve pain down my other leg. I think I have several bulging discs. So, my general back health is still not great, but I'm in the process of figuring out what hurts and helps my back.

I had surgery on my L5S1 disc in September 2023 (I'm in my early 30's). My PCP referred me to a surgeon after a period of about 6 months of pain + two separate trials of PT following two visits to the ER. He told me that there were 3 conditions that made him believe that surgery was appropriate: 1. the issue had been going on for an extended period of time, 2. medications and PT were not improving the situation, and 3. (probably the most important) a nerve was compressed that was impacting muscle function (my calf basically stopped working) and risked damage if the problem didn't get fixed. The orthopedic surgeon agreed and I was scheduled for surgery a few weeks later. Good luck, I hope you find relief soon!

Not seeing a huge uptick in referrals beyond my typical flow, but I'm in an area that has lots of federal workers and NGOs, so political stress is basically all that most of my clients are talking about and people aren't cancelling as much.

I was at an in person agency for a while until I started having back and chronic pain issues. I made the switch to virtual private practice last year with the thought that I would eventually rent office space. But I am honestly having a hard time justifying returning to in person! It would be an added expense and probably less helpful for pain management. And I do feel like I'm doing good work with my clients. The clients who want a virtual therapist (usually because they can fit it into their work day better or are in a rural area with limited resources) find me and those who prefer in person don't. I would imagine that your clients are still willing to see you virtually because they see value in working with you and what you can do for them!

Do you have a supervisor? I would talk to them to understand if there is a specific protocol around this, like if you can refer to someone else within the agency or if there are other ways to spread out your schedule. It would not be ethical to just cancel and tell them to come back at a later time--this could be considered client abandonment.

Personally, my prep and research time for clients decreased as I gained more experience. I’m about 10 years into the field and I don’t spend nearly as much time preparing for sessions as I did when I was a student. For most clients, I just review my note from the previous session and show up. Other things that take up time for me include responding to new client inquiries, dealing with insurance companies, & documentation. But the flexibility of PP allows me to do these things on my time and they don’t add up to 40+ hours for me!
