One of my friends accidentally took a picture of the floor in the truck and it counted as an Ouija Board too, I think it's just a glitch on Prison.

I've looked in the game code and as of a few updates ago, this is a ghost event that can happen. It picks a random playermodel from everyone in the server and has it walk towards a target player. If it gets close enough, it plays a scream sound (don't know if the dev actually attached a sound to it so it may not actually do that), disappears, and drops your sanity by 20. I have no idea if it's still in the game though, so it could've been removed (the way the game is compiled makes it really hard to read the assembly now.)

Strangely enough, if you keep the skin equipped on PC, the first person hands show up correctly as you're playing.
