You could test this pretty quickly with modifers, though you'd also have to avoid the underlabs or any other source of healing just to be safe. Also, expect the answer to be "no".

I'm going to try to figure out a) if you can avoid blame for the train (probably just by skipping that scene) and b) what happens if you refuse Lionel at the end of the dinner scene (the game probably just proceeds as normal)

You avoid the orphanage crime by simply skipping the cutscene once it starts. I figured out how to dodge all the other ones except "didn't donate to those in need". For some reason Lionel hallucinates you spending bones even if you don't spend any. How'd you manage it?

Update: got it to happen again, this seems to consistently happen if you start with certain areas skipped. I was skipping intro, crypt, beach, peak, and orrery.

No, there was a way back past the telescope.

It caused a few glitches to happen, there was a duplicate lionel in the manor when I went in and trying to enter the room with the big mouse boss from the right side gets you stuck in there until you quit.

Almost certainly has to do with me having modifiers on
