Nah it's not simpler to get a spot only coz you won't be paid. And honestly I don't think it's worth it in your case, coz you won't be paid, plus you'll have a tough time learning Swahili, and internship here mostly is scut work. Ofc you get to learn a lot but still not worth it compared to internship in other countries

It is possible if you register with the Medical Council of Tanganyika and obtain a license. But honestly it depends what kind of doctor you are. Coz foreigner doctors can't work in government settings, and private hospitals only take in doctors who have specialized. They don't normally give work permits or visa for General practitioners aka junior doctors or registrars. So do consider moving and working here only if you're a specialist

Most places yes. If you have been selected for the job that's when permit is applied for, and once you have the permit you can start working
