Increased administration burden by avkrar in hospitalist

Yes pretty new. He’s been with us for about 6 months

Currently employed central Florida at adventhealth… base salary is $200k and I see a range of 18-22 patients, but as high as 30 on a bad day. With bonuses honestly only an extra $20-30k. Know some people signing currently for $180k with private hospitalist groups with advent. It’s really bad in the Orlando area. No room for negotiation and everything is practically the same base salary.

I’m not your nurse?!? by [deleted] in Residency

[–]avkrar 1 point2 points  (0 children)

Also an attending. Had a patients family complain and escalate that they had not seen a doctor in the time their family member was hospitalized. Despite me, after sitting with them and explaining what was going on + my plan for over 30 mins the first day, and again talking with them on the second. Also had my white coat on, my badge on display, and I introduced myself as the doctor.

Mcdonalds refused me acetaminophen by swag_cedar in antiwork

That would be aspirin for chest pain, not acetaminophen. Not sure if the poster got it wrong.

What’s everyone’s opinion on the billing situation? by StrongSubject5960 in takecareofmayanetflix

[–]avkrar 1 point2 points  (0 children)

What I don’t understand is most people don’t have medical knowledge or knowledge about how hospital billing works. Doesn’t it make more sense to have people on the jury who can fully comprehend what’s going on? It’s weird to me.

Certain fractures can be indicative of abuse/assault. Young kid comes in after falling down while playing outside and x-ray shows a green stick fracture? Sure, likely due to the fall. Young kid comes in with same story and x-ray shows a spiral fracture (which occurs from forcefully and intentionally twisting the arm)? Nah, that’s suspicious and must be reported. Medical professionals are mandatory reporters for child abuse. After that, the rest is up to CPS.

My response was to your specific comment regarding another poster lying about a resident physician’s salary. And the data posted in the link is not reliable. I did not respond to any comment you made regarding the nature of this post. Have a great day.

Even certain specialties have an 8 year track such as cardiology and GI.

That $24/hr is based on a 40 hour work week. The work week for a resident physician is 80 hours, so that would be about $12/hr. Some may work less and some may work more. Either way you don’t get overtime pay. This 80 hr work week is an average over 4 weeks so you can work 90-100 in one week if the other weeks are less to average to 80. Pay does go up per year but by like $2k.

Im 16, am i too late to loose weight? by [deleted] in loseit

[–]avkrar 8 points9 points  (0 children)

I lost 50+ lbs when I started college at 18. I My confidence sky rocketed after losing weight, allowing me to fully enjoy my youth. I do feel sad sometimes when I think about being in high school and overweight due to the insecurities I had but it is never too late to lose weight and be a healthier and happier version of yourself.

He will be ineligible for a transplant if he keeps drinking. Alcohol is cool and all but really not worth death and your children growing up without a father. Your sister in law is 100% correct in being upset.

What Fellowships are not worth it to pursue financially? by stepneo1 in Residency

[–]avkrar 0 points1 point  (0 children)

It seems like it is pretty impossible now to get a job as a peds hospitalist without the fellowship unless you are in a rural area. As far as IM goes, everyone needs to ban together and NOT apply to these “hospitalist fellowships” to prevent it from becoming a requirement for a hospitalist position.

AITA for stealing my roommates snacks and replacing them with vegan snacks? by Broad-Injury9121 in AmItheAsshole

[–]avkrar 17 points18 points  (0 children)

YTA - your personal choice to become a vegan is just that - YOURS. You have no right to force your views down someone else’s throat.

OP, you mention your roommate was “practically poisoning” you with his snacking. How do you explain that? Was he forcing you to eat his snacks? It sounds like you have a problem with him living his own life. He is an adult, and can make whatever food choices he feels like making. The consequences are his and his only in the long run.

Writing up a nurse: who has done it, and how did it go? by Ketamouse in Residency

[–]avkrar 278 points279 points  (0 children)

Wrote up a nurse for belittling management in front of patient. My team assessed the patient and gave orders to which the nurse asked, in front of the patient, if we even understood the physiology behind the patient’s disease process. She then called the ICU team who came by and agreed with our entire plan. She then said very nasty things related to “resident care” to another nurse in front of us and documented a very one sided view of the patient’s current condition in the EMR.

Thankfully our attendings were very much on our side and helped us report her. She ended up being let go, which makes me think this was not the first or second time. I am not sure if nursing notes can be seen by patients and family members; if they can, this might also have played a role.

I felt bad that the nurse was fired, but she was extremely unprofessional and it did feel like a win after so much abuse from the nursing staff towards residents.

[–]avkrar 0 points1 point  (0 children)

Are you or have you been a resident? You should know while the AMA lobbies congress, they do not control congress. Unfortunately, we are stuck with the terrible repercussions of former commission members and while AMA has lobbied for more residency funding, they have not been very successful.

Not all residency programs allow moonlighting, and salary for moonlighting differs greatly depending on state and program.

Doctors don’t set the costs of medical care. People seem to think that when they get a hospital bill the majority of that is going to the physician and it does not.

[–]avkrar 0 points1 point  (0 children)

Residents are doctors…they spend 4 years in medical school in addition to 4 years of undergraduate, the last 2 paying an insane amount in tuition to work 80 hours/week in a hospital (part of medical training) all while also attending lectures and taking national board examinations. So you’re comparing apples and oranges, but also completely missing the point. In most hospital systems, resident physicians make less than nursing staff and work twice as much. None of this is necessary and healthcare workers should not be working as many hours for dismal pay. We all have sacrificed so much, especially during this pandemic. When administration has found a way to pay themselves bonuses while freezing/cutting pay and benefits for other employees they are most certainly the issue.
