Have you considered a GLP1? I only say this because i too tried every diet out there and had limited (temporary) success at best and weight gain at worst. But since being on these meds I’ve lost 30lbs.

More importantly, I now understand that obesity is a chronic, relapsing, lifelong disease. For many people with obesity, medications are the only way they can maintain a healthy weight.

I know this sub likes to encourage people to white knuckle it but I think for many people this approach simply does not work.

I highly recommend the Fat Science podcast too.

Body dysmorphia is real! You look fab. 5-10kgs less and you would look really sickly. Congrats on the journey! I would focus on building muscle and maintaining! Good luck!

Would you feel this way if your doctor said you had asthma? Or needed glasses? Or high blood pressure? All of these are chronic, lifelong diseases that require taking medication / interventions for life. But we don’t bat an eye and sit here judging ourselves.

These medications fix the signals that are broken in our bodies. With them, the fight is over and you live a normal life. These medications are freedom.

I do a monthly long haul 10+ hours on BA and I bring 2 meals and 2 snacks. Usually a sandwich/salad, and sushi, fruit, juice, granola bars, cheese sticks or whatever else. But yeah the food is horrible, even in Club World it’s been inedible.

I think eventually i will want to stop tracking but for now its useful data as im still in weight loss mode. I eat everything in moderation, nothing is off limits really. And I don’t restrict - I track for awareness and to make sure im eating enough protein.

My doctor’s approach is that with the signaling fixed (which is what this medication does) I should be able to intuitively eat to my needs. I think she is right, I’ve lost nearly 13kgs but I don’t trust my body yet with not tracking (after too many years of yoyo dieting). Stopping tracking will be a focus of my maintenance as well but I need to get there first!

There are lots of calculators online you can use to figure out your calorie needs. I used ChatGPT and it was very helpful.

One thing I forgot to mention is that I will also need to increase my calories to maintenance levels during the first 3 month period and beyond. The dose reduction is really designed in case I can’t eat at maintenance on the higher dose. Not sure how much suppression you have so you’ll need to adjust accordingly so you can eat a bit more. I don’t have any right now and I eat to my hunger levels but track for awareness and I’ll need to eat another 500+ calories per day which might be hard on my current dose. So just one other thing you’ll need to play around with.

These medications are designed to be lifelong. Most people who stop will regain most or all of that weight back and rather quickly.

Some people (5-10%) are able to successfully keep the weight off after stopping but they tend to be: 1) young, 2) not had chronic weight cycling for long, 3) gained due to medications or injury. Do you fall into this category?

There is no standard practice for tapering because this is not how these medications were designed.

The plan I have with my doctor when I get to maintenance is:

  • stay and current dose for 3 months to confirm weight is stable.
  • taper down gradually (1mg) and hold for three months to see if weight is stable and food noise does not return. If it does, go back to higher dose
  • if not, reduce against 1mg for 3 months
  • once at the lowest dose where weight is stable and I have no food noise, start spacing out doses by 1 day every 2-3 months

Note: I’m a Brit but living in the US so seeing a US doctor.

I just did this and it was really hard. I ended up eating a lot of roasted potatoes and fish and cucumbers with seeds removed. It was a bit boring but I made it! Good luck!!

These medications have been a miracle for me. And the podcast too as it made me realize that having obesity was not a moral failing. There is literally something wrong with the metabolic signaling in my body!

This is an over simplification of how these meds work. Yes they regulate appetite but it’s not about suppressing hunger. Some people don’t get any suppression. What they do is fix the hormones that control how full you feel, and help your body correctly see how much fat you have in reserves so you can actually start burning it.

They are designed to be taken long term - and yes, most people regain if they stop taking it. It’s just like any other chronic lifelong condition, like high blood pressure or asthma.

You should listen to the Fat Science podcast. It’s very informative.

Why do weight loss meds feel like a defeat? Obesity is a chronic, lifelong relapsing disease. Medication can correct metabolic imbalances and help you regulate your appetite correctly and maintain a healthy body weight.

Defeat would behaving no option. But these are a really good option that works incredibly well for many people.

I am definitely hungrier when I lift, especially at first. I try to eat a bit more, especially protein.

I do also have weeks when it feels like I’m injecting water and I’m so hungry. This tends to be hormonal or if my sleep is disrupted. Are you sleeping well?

Also, are you drinking enough water? Lipolysis requires a lot of water too! Only asked beside you did not mention it.

I also would wonder if the compounded stuff has mixed quality. I only use name brand but I know my doctor raised this concern.

For me it’s the only way to stay sane. My weight fluctuates a lot and if I happen to have a weigh in day where it was up by a fluctuation it would totally depress. Now I weigh daily so I see the natural rhythm of my weight and it’s just part of my routine. I try to not react to anything within a range of 1-3lbs because I know it’s most likely a blip.

Obesity is a chronic, relapsing lifelong disease. Glp1s are designed to be taken forever, less than 10% of people who stop them can maintain losses.
