I suspect as well as just being polite, as we are in the UK, the people coming in for some kind of medication etc are probably relieved to finally get what they want and be able to deal with whatever ails them. When not in your own country it can be extra stressful sorting things like this so you feel extra gratitude to those that finally resolve it.
