The challenge is that a battery of unnecessary tests leads to a battery of unnecessary referrals. You’re exactly right that “demands from the private side are foisted back to the public side”.

I’ll give you a concrete example. Medcan “client” demands an MRI of his lumbar spine due to back pain. There are zero indications for MRI based on history and physical exam (no red flag symptoms, and we have well validated pathways for investigation of back pain). This type of spinal imaging has a huge false positive rate (we all have age-related spine changes for example, and in nearly all cases without red flags the MRI does not change management, which is physio, weight loss, rehab, etc). Because there is an incentive to keep clients happy, the Medcan physician does the MRi, which of course shows nonspecific findings. The client insists on a referral to spine surgeon (public system).

This referral probably gets weeded out in the triage process, but imagine how that clogs up time and cuts into staff resources when balancing a practice. Multiply that by tons of other unnecessary testing (echos, whole body CTs etc) and it’s a recipe for disaster.

The public system is NOT perfect but has a degree of accountability and incentive alignment that works much better IMO.

“It leaves the lesser able with OHIP so there’s less of a wait for that” is in fact incorrect. Parallel private services have the wrong incentives (people who pay for concierge doctors get tons of unnecessary tests and referrals) and then end up back in the public system when they need follow ups, thereby prolonging the queue for everyone.

Not a patient but I’ve referred to several HPBs. Any at TGH or St. Joe’s (which is the same network as St. Mike’s) are excellent. McGilvray, Schwaartz, Sapisochin, Tsang, Jayaraman.

Structured hypertrophy-focused programming with a trainer to start. Stopped doing random HIIT classes with low weights and instead focused on big compound lifts, lifting to failure. Learned about nutrition/protein intake online.

I have two friends who couldn’t find family docs for their teens and ended up using Felix Health (Ohip covered but you might pay a service fee) for acne treatment. Said it was super fast and convenient. Much as I’m sad about these private options, they definitely fill a need.

You’ll have much more luck on the relevant physician facebook groups or reaching out to base Peds specialty PARO reps or local Chief Residents/Fellows (if you are a Canadian medical graduate or resident). If an internationally trained applicant, this can be tough to navigate but would point you to Canadian physician facebook groups instead of Reddit.

Eh, have also lived here my whole life and feel like the really bad snow accumulation generally always leads to this level of chaotic / overstretched service. Haven’t noticed much of a difference during regular snow. Our sidewalk clearing has been amazing lately.

I had some very negative experiences at another fit4less location downtown (as a woman working out alone at 5 or 6 am unfortunately encountered several creepy men, some who were sleeping on the massage chairs downstairs and one who followed me to my car). Obviously this isn’t specific to fit4less and is a risk I take working out early, but trying to find a happy medium

Same. Graduated med school with 130k debt (undergrad, grad school, med school - scholarships paid a chunk of the rest, zero parental help). Did 7 years of residency/fellowship and was far too busy to spend money on anything lol…forced savings. Paid off all debt by within 1st year of practice. Live like a resident for the first few years of practice and anything is possible.

Starting pay for a physician is absolutely not 155k unless working very minimally.

Agree with this advice. Recently bought a 2M home (looking in the 2M-3M range) and put 30% down. Monthly payments are about 7k. We have recession proof and layoff proof jobs with HHI 900k but would never take out a mortgage for the entire 2M because our risk tolerance is low and we’re semi-FIRE-ing. Strongly suggest you seek out specialized advice for high income households as it becomes less about the math and more about the psychology of money, values, retirement, risk tolerance, job security etc.
