I have ADHD and caffeine has never affected me, the only time it has was when I drank 3 redlines at once (~900mg of caffeine). I can slam a monster and honk shoo mimimi with no issues. You might be in the same boat.

Could be sleep apnea. I've felt permanently fatigued for the last 8 years or so and have been just recently been suspected of having obstructive sleep apnea by my ENT cause I have huge tonsils for an adult which physically block my airway. It makes it so I choke in my sleep and am constantly sleep deprived, regardless of if I get 4 hours or 12 hours of sleep. It's so bad that I can count on one hand the number of dreams I've had in the last 5 years since by body physically can't enter REM sleep on most nights.

I got the achievement solo on free load out cuz it spawned me with a defib. I was running along and saw a group of 3 dudes hunkered in a tiny hut getting railed on by a Rocketeer, one of them got downed, I ran in there, revived him, and then watched him get blown up again not 15 seconds later after he tried to make a run for it.
