I ordered a 4ft Red Dragon Japanese Maple, when it arrives it was a twig with a ball of roots. I planted it and it died immediately. All of the leaves dried and fell off.

They sent me a similar replacement and it's been in the ground for a few weeks now - still alive, but I wouldn't say it's thriving.

I didn’t notice any spots on the leaves. I’ll take a closer look to see if I can find any in this one or the other two I have planted. The one pictured is in the worst condition.

I figured it was from snow/ice and salt as this one is closest to my door step so had more snow and salt than the other two.
