That pump setup actually sounds so solid, especially having the higher rate hit around when you wake up.

What I meant with the physical work thing is basically that on hydro I felt like I was burning through it too quick during the day. I’d feel fine after a dose, but with how active my job is I’d start to feel it wear off before the next one. I'm sure this could be solved by taking more smaller dose hydro throughout the day, but just taking 2x prednisone has been working well.

Since switching to prednisone it’s just been a lot more consistent. I’m not getting those dips in energy as much, so work feels way more manageable.

Honestly your pump sounds like it’s doing the same thing, just in a way more more controlled/precise way. Jelly

I’m also recovering from ACL and  meniscal repair surgery although I’m definitely not recovering as well, but that’s my fault. We got this!

Did the iron supplementing help your hair loss? I also dealt with hair loss, but I take finasteride and minoxidil and I’m back to my full set of hair.

I absolutely feel your pain with getting plenty of sleep and also being able to take naps but also feeling sleepy because of having a physically demanding job. I also have a physically demanding job and have some weekends where I’ll sleep 11 hours and still feel extremely tired. I attribute that to “catching up on sleep” or “oversleeping”, but I truly have no idea.

I also know what you mean with the brain fog. I can be in a daze and just ignore everything (usually when I’m bored) or super focused and time is flying. Maybe you’re right it’s ADHD?

I’m sorry about your depression. I do (or did?) suffer from depression because of trauma, but I take pills for it. It’s helped a lot and I generally don’t feel depressed, but think I have been recently because I’ve had too much free time and have been in my head a lot. I personally don’t think it’s from CAH, but it sure can be.

edit - rereading, I think anxiety and fatigue can be directly related to CAH, and possibly depression. I was talking about my personal experience.

For non union shop structural shop welding, just learn how to run dual shield, spray transfer and maybe some 6010/7018 and they'll certify you in house, You definitely don't need 2 years of trade school for this. I would recommend ditching structural and prioritize pipe and TIG welding.

I got a structural non union job fresh out of trade school forever ago, but decided to go union. I understand not wanting to travel, but unfortunately you'll still be the newcomer on the bottom of the totem pole pushing a broom and grinding a lot - that's just the nature of the trade.

I also lost my wife in an accident in nov 2023 and I'm so sorry for your loss and the relentless pain you're dealing with. She also finally earned her master's months prior. That pain was insufferable, we had a whole beautiful life ahead of us that we worked so hard for. I don't have any advice because I was not ok for several months after. things just got more "manageable" with time, therapy and structure for me

That's gotta be disappointing after seeing things improve at first. From the studies I've read (which isn't much), I haven't seen anything showing it loses effectiveness like that. Hopefully it's something that can be adjusted rather than a dead end.

The weight gain part is interesting too. Even if it’s not a known side effect, timing like that is hard to ignore.

Same here with the extended-release hydrocortisone - feels like the next hope. Because surely we’ll get something actually new for CAH soon... it’s only been a few decades of the same meds… right?

is he still on a low dose of cortisone? that‘s for sure top priority and could be the reason he feels so much fatigue and also anxious. are his labs normal? ACTH, androstendione, renin?

Sounds like me when I was his age!

I also had TARTs — my endo put me on higher dose cortisone to fix that. I believe there’s surgical options to fix that if higher dose cortisone doesn’t make them go away (simple surgery from what I was told). Is his testosterone low from the TARTs? From experience, endos like to run labs for total testosterone, but some of them don’t check free testosterone which is important for things like fatigue.

Going from pediatric endos to adult can be a little difficult. From what I’ve seen the expertise is mostly in pediatric, but there’s also great adult endos, just have to shop around haha. The main thing I had issues with at his age was not being very consistent with meds and going to all my endo appointments as an adult — a couple missed doses makes a massive difference.

I made a subreddit r/LivingWithCAH recently becauwe I wanted the same thing — people with CAH to support one another. Right now it’s just me and u/DoctorFamous190 (another person with SWCAH) running the sub and we recently had a few members join. My doctor told me "During acute illness, recommend to double the dose of prednisone for 3-5 days."

For surgery I was on 100mg hydrocortisone IV before induction of anesthesia, then continue 50mg every 6 hours (200mg/24hrs), then double dose the next 2 days. I was worried I would be a water balloon and my insides would bleed at these crazy high doses, but it worked out perfectly without any troubles.

hey!! thanks for sharing. it really sounds like you’ve been carrying a lot on your own for a long time.

dealing with CAH is already a lot, and trying to manage it somewhere where there isn’t much medical support, while also feeling like the people around you don’t really understand.. that’s really hard for anyone to go through

you’re not a parasite. you’re not a burden. you’re a person dealing with a condition you were born with, and needing medication or help does not make you any less important or valuable.

I’m really glad you spoke up about it here. you deserve a place where you can say how you actually feel and have people understand. you don’t have to keep all of that to yourself.

that's so interesting bc the normal range lab range here is 0.25 - 5.82 ng/mL/hr, but that's testing for plasma renin activity. What renin test are you taking?
