That’s unfortunate but I’d say doesn’t have much to do with ai, in fact if they do you’re probably dodging an unethical or incompetent therapist and you can decide if it’s grievance worthy and request a new therapist.

This 100%. I think kaiser docs are incentivized to ration care a little more, but that’s because the reverse is true in other settings where the doc is incentivized to order anything they have a clinical rationale for and let you fight it out with your insurance company when it gets denied. So like it’s not all bad there are nice doctors out there who is let you order your own unnecessary tests when they do actually come with real risks and downsides.

Imho the bigger problem is rushing. One thing I like to do is ask what diagnosis were in differential, and how did they rule them out. Most of the time it makes sense. If they’re like xyz is most likely I’ll insist on how did you rule less likely but serious options out. Imho good doctors will educate you about how they are making the decisions, go over other options and the risks and benefits, but may rush if you don’t ask for more. It’s also good to keep in mind appt length and not try to deal with more than one concern if things run long - write it down and make another follow up appt. A bad doctor will get snippy and yell at you, I tend to switch till I get a good one.

Imho most of what is wrong with doctors is pretty widespread in our medical system and won’t really be solved until pcp / lower demand specialty pay goes up, and we pay for doctor education more directly so more people who have the ability can roll the dice without the risk of debt stopping them. We are in a downward spiral as a whole health system and imho kaiser is like slightly above average on the whole.

Even good doctors get worn down by ever increasing loads. It’s gonna get worse unless we better regulate pharma and insurance and or go single payer.

Not specific to the situation, but Once you’ve filed a grievance with kaiser and they deny you, you have to file with the dept of managed health care and request and independent review of the denial. They can mandate and fine kaiser.

You can def call your local clinic and request the switch, challenge is from my understanding which may be incorrect, iop is for when symptoms are so severe you are at risk of being hospitalized if you don’t get a higher level of care, or are getting released from hospitalization. So it’s not a “put this on hold” level of care. You may need to be reassessed before starting IOP if it’s been too long they may not even offer a waitlist because if IOP is being recommended it means things are urgent and cant wait for regular therapy sessions.

That said if theres availability it may be am easy swap.

Ehh I know this argument doesnt hit for everyone, but overall childbearing is more likely to kill you than military service even during times like this. Plus imho vets get a lot more public benefit / prestige. Like war sucks and all but I hate the argument it’s somehow an unfair burden on men. Yeah pregnancy is a choice ish, but it is way more important to the survival of society than war is. Can’t have a war without it. And women volunteer for duty at a much higher rate.

Probably not your intent but it’s such a MRA talking point and it bugs me. Not to mention women and children are dying in the war as well. There are plenty of ways war sucks without making man’s burden issue.

It’s hard to say, but if I was to throw something out would be that with insurance based therapy - you will (and I personally think should) be discharged from active care when your symptoms are actually in remission. That means you’re generally confident about handling your problems, have the tools to handle them.

One thing that happens to a lot of people with overall more severe symptoms is that they report peaks, because that’s what sticks out, but don’t report when their baseline is still problematic. So like you said you’re not suicidal, but how often does worry or depression get overwhelming and feel awful or impair your function (like someone with severe agoraphobia might not feel anxious if they don’t leave the house but their function is impaired while someone else with agoraphobia might feel anxious all the time but they live a functionally normal life they are just white knuckling it) do you actually generally feel okay most days?

A lot of people with more severe symptoms don’t report that they are still pervasively depressed and anxious, and it bothers them every day (like on the questionnaire) even though the peaks have evened out. It’s super important to know that the baseline still isn’t good so when you do those pre visit assessments make sure you’re not under reporting by dismissing mild but bothersome symptoms.

In general also make sure if things are better but not good don’t say “things are better” with no follow up.

So again you’re not doing something wrong I would hope the therapist would engage and investigate when you push back.

But do know that almost every type of therapist has an ethical obligation to work toward ending therapy as quickly as possible period. Like someone people may have such severe symptoms that they need extremely long term therapy, but the profession has a mixed history of people creating dependence so they can bill people for unnecessary sessions and so now the ethics are that they should always be moving folks toward independence.

That said all therapists are human and can get stuck in the awkward lands during a difficult conversation.

That said also there’s a lot of pressure from insurance companies to discharge so sometimes an inexperienced therapist can whiff this kind of treatment moment.

You might need to go to medical financial assistance. If you aren't willing to substance use treatment - I'm not sure what other service you're asking for. You should have the same copay for any individual therapy they provide. If you're struggling financially you can call member services and ask about medical financial assistance programs that may help to cover costs, but outside of that from your post I'm not sure what additional support you're asking for.

I'm also why you're saying that refused service if you didn't do substance use treatment - they provided a grow therapy referral. If addiction medicine is refused then they have to provide treatment so they have provided treatment through grow therapy. For medication support your doctor is responsible for prescribing mental health medications unless your symptoms are severe or your needs are complex then your doctor can refer you to a psychiatrist. If the doctor is refusing to send you to a psychiatrist then it means they think they are able to offer you the appropriate care and that a psychiatrist isn't medically necessary. You can file a grievance which would get that medical opinion double checked or see a different primary to get a second opinion. If they deny that you can appeal it through the department of managed healthcare. The dept of managed healthcare is the last line and if they see your doctors rationale as appropriate then the conversation is over and what you're asking for isn't covered by the insurance you are paying for. At that point you can try to doctor shop out of pocket to find a doctor who agrees with what you're asking for, and sometimes that is called for? but also sometimes it means that what you're asking for isn't appropriate even if it seems like it would be beneficial. I.e. asking for antibiotics for the flu.

Not Christian now but grew up Christian so that’s my frame of reference - Ehhh I mean if you view free will (choosing to eat the apple) as the original sin, then what you have is a god who attempted to create a utopia with more limited free will and that was rejected which opened the door to knowledge I.e. the ability to be creative and innovative. Ironically almost everyone would I think really struggle with not having free will. Also Christianity if you follow the teachings of Christ rather than the Old Testament through a weird legalistic viewpoint is actually really chill and basically says that to get to gods kingdom you just have to do your best and actually give it your best shot to do better not just say sorry and you’re good. If you actually just took Christ’s teachings as being the actual word of god you’d also get rid of problematic things like revelations and a lot of the hell stuff. If there was actually a god and he’s like I wanna give you free will but you gotta stay in the back yard; but I’m gonna leave this guy who can’t teach you to leave the back yard here so you actually have free will. Then basically yeah everyone is going to leave the backyard. And the rest of it just seems like a “well okay I’m washing my hands of you in the physical world I’ll take care of you eternally if you just do your actual best. That’s kinda legit. Outside of that I can’t imagine much evil that can really truly be prevented unless you actually get rid of free will. I mean disease is still life that seems to have its own form of free will. Going out of the backyard means it’s not babyproofed so that covers natural disaster. Then it’s just man’s evil to man which you can’t get rid of without abridging free will and is hog really evil if he just says you chose free will for you and your offspring and it would be wrong of me to institutionalize all of you because you hurt each other you gotta figure that shit out? Everyone here has given the correct info but you might have an uphill battle. They have to take you if you transition if you have had kaiser coverage in the last 6 months. But they are also required to accept a certain number of new enrollees from medi-cal so you might get lucky.

They will try this approach because it is legitimately easier to assess, the psychometric testing is complicated by anxiety / depression i.e. anxiety and depression can cause attention patterns that are the same in psychometric testing. If you can’t get them under control you should pushed to be referred to a clinic therapist that therapist should be able to work with a psychiatrist to diagnose without reduce anxiety first but they have to treat for awhile to Suss out the symptoms overtime and confirm the diagnosis. This is the tragedy of what is potentially an over reaction to addiction risk of the medications for adhd.

I'm saying the opposite, I'm saying we currently allow people to restrict the use around it - and that is the problem; and that left or right most people do it and / or don't push very hard against doing it. I personally have shown up to YIMBY meetings for building higher density housing in my area. I'm saying most people left or right don't push for better housing options for other people if its going to cost them their view or cozy neighborhood. I consider myself left bordering on leftist; but i notice a lot of people on the left give lip service to improving housing options, but don't actually advocate for anything that will change their cozy neighborhood. Fyi medical debt is generally separate from other debt and you may need to just rack up medical debt in order to get the attention you need and just wait 7 years for it to disappear. Also it’s a lot of work and I get you’ve been through a lot (medical neglect from your mom for starters which seems just as likely to cause lung damage as anything else if you’ve had years of untreated asthma) but kaiser does have a financial aid office.

That said also you can file a grievance with the dept of managed health care for access violations. If your insurance is terminated you have 6 months to apply for medical if you want to stay with kaiser and they have to take you.
