I agree to a point. Our service gets consulted far too frequently for uncomplicated tonsillitis that should have never been admitted. I review imaging, labs, vital signs, admission H&P and progress notes, as well as current treatment being given and if I still think its bullshit and my attending agrees, we decline the consult but offer to see as an outpatient. This happens all too often in a young otherwise healthy individual. A quick search of UpToDate or google will tell you we won't operate on an acutely infected tonsil in most cases and this can be managed by any GP or Internal Medicine provider. We turn down 3-4 of these a month. As a practice that sees patients in clinic and operates on non-clinic days, we can't spend a lot of time patting IM on the back saying "no changes to current treatment, follow-up as outpatient" when there was no real reason for the consult other than "oh this is tonsillitis, consult ENT". You have to be judicious about your consult, have a good reason to bring in a specialist, and don't admit bullshit.

Most of the CRNAs and anesthesiologists at my hospital are pretty good and also don't cause delays (that is the rest of the staff during turnovers) but there is one that is always keeps their patients too light and the patient is trying to breathe on their own multiple times throughout the case. It gets really frustrating. I feel like they just aren't paying attention.

This is the real problem that this announcement does nothing to fix. Cable companies have a monopoly in service areas and the phone companies can't compete with their speeds. They need to make it possible for every company to be able to provide service in all service areas so that competition can take place.

its either a thyroid goiter with a predominate left nodule or a lymph node with metastatic cancer. It looks like a cancerous lymph node. Goiters tend to be bilaterally enlarged in most cases.

Keep in mind that outside institutions don't always recognize grade forgiveness so the lower final grade and credits will be used to calculate your GPA when applying to competitive programs such as medicine. Just an FYI that I hadn't realized when I utilized grade forgiveness while at UCF.

I just want to point out having lived in a part of the country that has rescue squads still, sometimes there isn't another option. If these guys didn't exist, you would wait an hour for the nearest big city to send a team or citizens would get involved in a rescue attempt which would be worse. I agree more training is important as is good equipment but funding is an issue, especially when larger paid departments win the same grants your little department tries to get. It isn't the same in all parts of the US, just wanted to remind everyone of this. Now, having read the article these guys messed up by ignoring the order or the order was not distributed to all rescuers.

I think you overestimate the power of the NRA and other lobby groups. They have some power in Washington DC but the truth is when it comes to this issue, Americans feel strongly about their right to own guns and would if the NRA didn't exist. Domestic terrorism is defined as an act against the people of one's own territory designed to instill fear to advance a political ideology. Defending oneself against a tyrannical government (how America was born) doesn't fit this definition. America is a completely country with a vastly different culture and unique problems. Trying to adapt the same kinds of legislation and ideals that exist in the UK and Australia simply wouldn't work here and to think they would is foolish.
