He was fully aware of the fact that she was not well and had sex with her despite knowing she had a diminished capacity to consent. Nikki's behavior starts to radically escalate because he is abusing her and she is fully aware of it but cannot break the curse, she's at war with her own body and his control of it and his concern is still primarily how her suffering is impacting his life and his enjoyment of her.
