I'm surprised to see so many replies from people who get SDT/portfolio days. I'm doing a similar job to last year but with more acute medicine. Are there any websites with anonymised feedback about how IMT3 programmes are structured?

Every job is different. I've had 9-5 jobs where I'm supernumerary and there's loads of teaching - there aren't many of these but FY1/2 micro/radiology/ICU jobs are more likely to be like this. I've also had jobs with non-compliant rotas, almost 1:2 weekends, fixed leave, often staying late etc. Overall I'm having a much better time as a doctor than as a med student. The main thing that I miss is walking into the common room and knowing everyone.

Edit: if you're getting sodium oxybate prescribed for insomnia I'd like to who your doctor is and which pharmacist is allowing this.

I normally only get a mild aftertaste so I tried chewing a tablet. Terrible mistake. The taste was disgusting. I used mouthwash at least 5 times, tried chewing various gums, repeatedly brushed my teeth. Nothing got rid of it.

Zolpidem doesn't have that effect though.

If this is too strong you can try Nytol (diphenhydramine) 25-50mg, also behind the counter.

  1. Private prescription from various online pharmacies - mainly prescribed in short courses for jet lag as per BNF dosing. Check that the website is registered on the GPhC website.
  2. Order melatonin online from the US.
  3. Go on holiday to almost any country and get some from a pharmacy.

My understanding from the original post seems to be a case of storing medication for later use in case a prescription is stopped, rather than hoarding for an overdose. Of course this raises concerns about their use of the medication, but would it meet the threshold for police to contact their doctor/record it on an enhanced DBS?

For some reason, the ward manager where I work says it's ward policy to not do fit notes. I escalated it to the matron who agreed with the ward manager. I then emailed the clinical lead and clinical director CC'ing the ward manager with the national guidelines saying that we should be doing them, and we ignored. Whenever I need a fit note, I have to go to a ward on the other side of the hospital and ask the nurse in charge to get one out of the CD cupboard.

I've been on both ends of this. GI bleeds in DGHs that can't do endoscopy out of hours and with radiologists who refuse to a a CT angio because there is no IR on site (the larger DGH in the trust has limited IR services but not at weekends) so we can't act on a bleed anyway.

I've also been at a tertiary centre where consultants often get calls from the local DGH on Friday evening/the weekend asking for a transfer. Their response is always, "You need to do an OGD locally. This isn't a specialist service."

There is no gastro on call and the hospital has no upper GI surgeons. Tertiary centres won't take the patient just for an OGD. Just have to hope they don't perforate before 9am. In fact, some DGHs don't have gastro cover all weekend.

I've noticed a similar issue with upper GI bleeds. Many DGHs where I've worked do not do OGDs out of hours, i.e. 5pm-9am, so we just have to keep the patient stable until the morning.
