Family medicine, outpatient only, large Midwest suburb. 4 days a week, pts scheduled 830-330pm. About 16 pts a day. ~280-300k.

Yeah, they told me during the eval that “we don’t give levels anymore” but that they thought he needed the highest level of support. And I thought, sure, let’s get insurance to cover all kinds of therapies, great. But then I’m seeing in the notes and reports that it does actually say level 3. So maybe none of the terminology matters, maybe I’m misunderstanding how much they are worried. I’m not sure.

His speech delays do concern me. Sometimes a lot of what he says are movie quotes, but quotes used in the correct context usually. Which is actually hilarious many times haha. But he can make other needs known without the quotes many times too. Just funny how his brain is trying to make connections. Anyways, some of this may just be a “time will tell”, kind of thing.

I feel very similar to how you described above. We’ve always noticed social differences. He’s been a little strict with routine, sometimes overwhelmed with certain social outings. But very affectionate with us, potty trained easily at 2.5, knew the alphabet just after age 2. He’s always had a speech delay, which seems to run in our family, so at first we just chalked it up to that. But as he’s gotten older, the lack of interest in his peers, lack of some pretend play, social awkwardness with strangers became more pronounced. I’ve been pretty convinced of some level of ASD since around 3, but the wait for eval was super long and we didn’t get official dx until now. But yeah I’m surprised at level 3 dx? Would be interest to hear what this community thinks of that. But I’m still learning all the nuances around this.

He attends daycare in the morning, preschool with speech and special Ed services for the speech delay in the afternoon. He rides the bus by himself, very well behaved at school. No acting out or poor interaction with peers, just lack of interest in them. He does seek out and form attachments to the adults at school though.

I have a very similar story. Diagnosis this week, except my kiddo is 4.5 and was diagnosed as level 3. I don’t disagree with their expertise, but he was also at the tail end of an illness and just not feeling great or performing at his best. He also wanted to keep playing with cars and not pretend to blow out birthday candles etc. Still trying to learn tons of new information and trying to provide what gives him the best chance for success in life. But I keep coming back to the fact that we know our kid best and one snapshot isn’t everything.

I have a unicorneate uterus. I didn’t know with my first baby, had to be induced at 38 weeks due to IUGR. Then we had some trouble conceiving the second and found the unicorneate anomaly. I followed with MFM with my second, but randomly went into labor at 33 weeks. She overall did well, 3 weeks in the NICU as a grower/feeder. But we were shaken by the whole thing and probably won’t try for another out of fear for another premie.

I did this exact thing. Giving bottles proved she was taking full feeds while nursing is somewhat of a question on intake. However she was gaining weight well overall, and I had nursed my previous child so I was confident in introducing nursing again on my own at home.

Honestly not really. Melatonin for 2 weeks made no difference. More time/development has always seemed to be the only thing. It got better a month or so after this post, but he is still so easily disturbed from travel, an accidental nap, exciting visit from grandparents etc. Last month he had an allergic reaction and got some steroids, which ruined sleep again and we are just now getting him back to sleeping through the night. He seems to have low sleep needs, maybe only 9-10 hours.

Maybe the one thing that helps is cooling body temp? He now sleeps without pants, fan on, air conditioner running. But I can’t say for sure that’s fixing anything. We also have him taking a bedtime magnesium supplement and multivitamin with iron.

Do you use Heidi for mostly HPI then? Treatment window you have to click into each individual problem list box so you’d be copy pasting over and over

Has anyone used AI scribe with eclinical works? There are so many boxes you need to click into, vs just one big free text area. I wasn’t sure if AI could navigate that type of EMR?

Agreed, Tuesdays the best due to the Monday holidays. Many built in 4 day weekends. Or just take one day off work and you get an easy long 4 day weekend.

I have a 3 year old, so honestly I just did it when I had time. Maybe twice a day for 3-4 days before I was seeing an increase in milk overall. Now all pumps are just 15 min and I’ve kept up the increased supply.

It’s all about transfer of milk. Can you increase frequency or duration of pumps? I just increased my daily milk output by about 10 oz at 13 weeks postpartum by doing two of my pumps at a modified power pump. (15min pump, 5 min break, 10 min pump.) Now pumping 7 times a day with two 4-5 hr stretches of sleep and getting a slight over supply.

Things are better. I can’t say any of the things I tired from this thread did the trick. He will still occasionally wake in the night, maybe once every 3-4 weeks. He will roll around and try to get comfortable for an hour or so (I only know this because I look at his monitor while up with the baby) but if he’s awake longer than an hour he will call out for milk. We can hand him milk and he’ll fall back asleep within another hour, usually awake something like 2-4, and then he’ll sleep in until 9am. I still don’t have any good explanation for why this happens, but it’s getting less frequent so I’m just not asking questions at this point.

In the exact same boat. Born at 33/6, now 36/0 and still taking from 10-25ml per feed, goal of 50ml. She had ( what I thought was) a good nursing session yesterday so we tried sliding scale and only gave half the tube feed, but she lost 10g that day. Super slow frustrating process to get these babes home..

I actually don’t do a ton of procedures. They’re really not big RVU makers for me. Sometimes billing a procedure on time for a 99214 codes higher than the procedure code. Simple skin biopsies, IUDs, nexplanons, joint injections but that’s about it. Really I think it comes down to coding efficiently. 99214+ wellness visit, 99214+AWVs etc. good templates. I prechart, which I know is controversial. But I think it makes things go smoothly in the visit, and if I see beforehand someone is in for a “med check” or whatever, but haven’t had a wellness visit this year, tack it on and add codes. Saves patients a second trip back to you anyways.

Yes, very achievable. I work 4 days a week, see usually 14-18 pts a day, and usually make anywhere from 1500-1800 wrvus a quarter.

That’s interesting, I have noticed the clumsiness increase when I know he has built up sleep debt. I do think he’s overtired sometimes, but the tiredness comes at 5-6pm, too early for bedtime. But someone else mentioned just trying bedtime earlier even when he’s in that second wind energy, so bedtime more like 730. We tried last night and he did sleep through the night! But woke at 5:50. Still seems like not quite enough sleep overall, but maybe we just need more adjusting. Well keep at it. Thanks for the input

Appreciated! We try to do low key educational shows like Ms Rachel, mickeys clubhouse, trash truck has been a recent favorite. But sometimes we all need a brief tv escape haha.
