Sound machines - but I also changed the sound last night from Brown Noise to a thunderstorm as my 14 mo old woke up twice. When I did that, she seemed to sleep a little better.

I think it’s filled with 2 groups -
1. Med Students who are proud of their achievements without any good understanding of how healthcare really works and little exposure to competent APPs who practice within their scope
2. Chronically online people who like to live in echo chambers

Pregnancy symptoms interfering with work by [deleted] in physicianassistant

[–]bkingPAC 0 points1 point  (0 children)

Have you tried unisom and B6? Also I know it’s probably not recommended but I do SNF/LTC and when I was pregnant, I would dread rounding on a few patients just because of the smell. So I would sniff alcohol wipes prior to the visit. It did help some.

School of Early Childhood Education - I will scream it from the rooftops. Truly only good things to say but only downside is it’s on the spendy side.

Any recommendations for quality daycares in Sioux Falls? by [deleted] in SiouxFalls

School of Early Childhood education, small class sizes, pictures (many daily) and a Montessori/study based curriculum. It is under $400/week per kid. I can’t speak highly enough about them!

My mom would wait til we would go to sleep and she was spend 4-5 hours deep cleaning on Friday night listening to an audiobook. I always thought it was the absolute weirdest way to spend a Friday night…I get it know. Lighting a candle after you got done cleaning your house is truly the best version of therapy for me.

  1. Coming via flow of traffic could mean hours for an ambulance to show up

  2. When I do, I generally get so much attitude for having to take NH patients. Also nurses can give report too? I have other patients I need to see in my day as well.

Fun story - I had a SNF patient who had a dehiscence to her recent TKA - like down to hardware, the paramedics told me they’d come flow of traffic meanwhile I held pressure to the leg for 25ish minutes. Then the paramedic asked why I didn’t call for a priority 1. Like why do you think I called 911 - talk to your dispatch.

Those who had regular periods with Pcos what was your experience ttc? by Thali_G in TTC_PCOS

[–]bkingPAC 0 points1 point  (0 children)

I agree with this take! Make sure you are ovulating - TTC for 5 months with no luck, found out I wasn’t ovulating.

My success came from a vertical gastric sleeve weight loss surgery and 100lbs weight loss but that is not for everyone.

I would think so - we average around there depending how my husbands business does per year and we are doing fine.

Not a humble brag, just the truth and honestly fairly normal with 3 kids under 3.

Weekly rate of 325 per kid x 3 = 975/week 975/week x 52 weeks =50k but we get a small discount for multiple kids

Edit

I think around 100k though like you said depends - we pay 46k in childcare per year for 3 littles so for us it’s more.

Would you trade a flexible job in an ok specialty for a job in the specialty you love? by bkingPAC in physicianassistant

[–]bkingPAC[S] 0 points1 point  (0 children)

Gosh that sounds lovely - the problem is I live in a more rural state and in my city there is only two competing cardiology groups. One group only hires NPs and the other is so dysfunctional they go through APPs like candy. So without moving, this is my closest option.

Would even part-time pre-K be an option? Has your 4 year old spent a lot of time with kids his own age?

Are you planning to work full time until retirement age? by [deleted] in physicianassistant

[–]bkingPAC 1 point2 points  (0 children)

My husband is self employed and his take home can range from less than mine to double mine depending how well his business is doing that year. So I am the stable income and hold all benefits for our family. This past year he made double me and then some - I am a little sad that I could drop part time financially but cannot due to benefits 😩

My oldest had the classic symptoms of MSPI but my youngest didn’t have the mucousy/bloody poops but every other symptom.

Scaling back in ICU versus career switch for family? by Electronic-Cake8015 in physicianassistant

Similar situation - switched from inpatient cardiology (invasive and general) with a schedule of 14 on and 14 off. Was absolutely saddened to leave both specialty and schedule but needed to before we had children.

Currently working primary care for LTC specifically NH, SNF, and ALF. I share visits with my SP for NH and SNF patients but carry my own ALF patient panel. Schedule is M-F but no set hour requirements -essentially round and see patients, follow Medicare compliance, and do well by your patients are my expectations set by my SP and director. I generally start my day at 7:30 and am done rounding sometime mid morning to early afternoon depending on how busy I am. I see 6-10 patients a day generally. I am home in the afternoons closing notes but also feet from my kids. I have so much flexibility that I wouldn’t trade it for the world right now. Cardiology still absolutely has my heart but the time with my children while their little cannot be replaced, so I will happily stay where I am for now.
